Insulin resistance? Different insulin needed? Help!

I'm wondering what is going on with my boy. I got insulin from a new compounding pharmacy, and I am starting to wonder if he is just not going to respond the way I'd hoped with the PZI, and if we should switch to levemir or lantus (can't remember which new vet uses, I think it is Levemir). I don't think he responded well to the sliding scale I tried using. How will I know if he is resistant? Vet test? Jack absolutely hates going to the vet (he gets really pissy and mean, and he is not that type of cat. He's an absolute lover!) so I want to prepare us both. I updated his SS, so feel free to give a look. I don't know what is going on with him and these freakin' numbers, man!
It is hard for me to get good PM numbers, so there's a lot of gaps in the evening. I'm trying to get better with that, but with my schedule it is difficult.
Any suggestions are much appreciated.

He really does bounce all over the place. It is hard to tell exactly what it going on - I do think you might be changing the doses too often and by too much at a time. There are some good cycles in there. On 12/27 he went down to 123 on 1.1 units and then bounced back up to the 400s. On 1/11, he went down from 326 to 194 on .8

So I think he is not insulin resistant. He is responding to the insulin, just not in an organized way. He either bounces or it doesn't last the full cycle sometimes, but it is hard to tell which.

Yes, you can certainly try another insulin and it may smooth out the drops. If you change to Lantus or Levemir, be sure to read how to dose it. It is very different than ProZinc and I'd suggest you get some guidance for the first few weeks at least.

If you want to stick with ProZinc, I'd suggest starting to post daily with his numbers so we can advise you on possible smaller dose changes and letting him settle into a dose if it looks like it is doing something. There are also ways you can use food to smooth out ProZinc curves and maybe smooth out those bounces.
 
Thanks Sue! It is really frustrating. I think both the bouncing and insulin not lasting as long as it should are happening. I don't think a sliding scale is going to work for him, which is kind of what I have been doing. I will start posting every day. Thanks!
 
To get the color coding in the new cells, click a cell where that is working, then click the paint roller icon to paint it on to new cells.

You might try stabilizing on 0.6 units for 3 doses, no sliding, and keeping consistent times. Then check the nadirs and see if you can go up to 0.8 units. OR, if your schedule allows, do you think you could shoot about every 8 hours?


I've used both ProZinc and Lantus ... and R and N when I had Spitzer. The Lantus is very picky about doing dose adjustments and cannot be adjusted on the fly. The ProZinc didn't last long enough for him. I used the R and N insulins to pull down the glucose level on Lantus when I couldn't increase due to intermittent vomiting and resulting low glucose. I finally got to the place I used Lantus at a baseline dose, then added the ProZinc on top of it when he was higher. No, not an established protocol and you should have seen the posts in response to me doing that! It was starting to work and then he threw the blood clot and I let him go.

Thanks for the dosing advice. Do you think I am giving him too much insulin?
 
I tried your suggestion, but the paint roller icon didn't do anything, BJM.
I don't know why it stopped giving the corresponding colors on the ss :(
 
Checking - you looged in to Google Docs/Drive first and then opened your spreadsheet, instead of clicking your signature link?

There is conditional formatting for the cells, ie if this, then that.

It should work, when logged in, to click a formatted cell, click the roller icon, then click an unformatted cell to paint the format there.

It might work to click an entire row, then click an unformatted row.
 
Yep, logged in to Google then clicked onto my ss. Not sure what the deal is with my computer at work. Will try again on the ipad when I get home.
