Insulin not really working

My cat has been diagnosed 2x in the past, both times went into remission. He is now 12

Once was about 2 years ago, he had DKA, pulled through and then after a couple of months was in remission

Last year, he was diagnosed again, and within 3 weeks, went back into remission

On both occasions, he was on .5 unit of Lantus 2x a day.

This time around, he was on .5 unit of Lantus for 7 days, still reading in the high numbers 20-HI

For the next 10 Days he was on 1 unit of Lantus, still kept reading high, the numbers more 19-HI

Now he is on 1.5 for the last few days, and still reading HI. Outside of a couple of times dipping down to 18, he is still mostly reading HI.

For the last 2 injections, he has stayed on HI without the dip after the injection. His last inject had a slight dip to 21 about 2.5 hrs later, but otherwise just 21.5-HI (we are about 3.5 hours after last injection now)

To note, he had a new Libre2 sensor put on about 18hrs ago

I'm told to stay with this 1.5 unit 2x a day for the next 7-10 Days

When your cat was diagnosed, did the vet do a set of labs to rule out any medical problems? What about any dental issues? If there's an underlying infection or inflammation, it may be keeping numbers higher.

It also takes Lantus about 5 - 7 days to build up in your cat's system. You may not see a great deal of movement until the depot has stabilized.

I'd also suggest trying a hand held meter. The Libre can be a bit twitchy at times.
