We got back Binkys bloodwork today his white blood cell count is 27 thousand. The vet said he has a systematic infection but doesn't know where. She said he is in the beginning stages of kidney failure but doesn't have a UTI. His heart enzymes are off, and liver ALP 94. She wants to do a ultrasound on tuesday but what worries me is she didn't give him a antibiotic, i mean if he does have a infection should she have given him a antibiotic?
 
I am going to tag @Suzanne & Darcy, she has much more experience with a wide variety of health issues than I do. High WBC does not always indicate infection...

Now if something is off with his heart, I'd be hesitant about the ultrasound - most cats needs anesthesia for an ultrasound, but that can be risky if a cat potentially has heart issues. If it were me and I could afford it, I think I'd see a cardiologist first for a consult.

Can you post the lab results you have? Images are perfectly ok, just scrub out any personal info.

yes i will try to post the results i really need some advice i'm so stress, my heart is racing,

OK, first of all with a white count that high, I do believe that your vet should have prescribed an antibiotic. It appears that there is an infection. I have been through this many times before with infections of unknown origin. If it is an infection, it will soon respond to a broad-spectrum antibiotic most likely. You would be able to confirm this with blood work in a week or so or shortly after ceasing the antibiotics. If it is something other than an infection, the white count would not be reduced upon rechecking the bloodwork.
 
Also, the absolute neutrophils are elevated - again indicating an infection. Creatinine is good. BUN elevation may be due to overall dehydration.
 
Regarding an ultrasound, my cats have never needed to be sedated for an abdominal or cardiac ultrasound. I suppose it depends upon the cat and the practitioner.

SDMA was 22 but can that also be dehydration?

Oh what it would be like to have a cat that is nice to the vet :rolleyes:

(Sort of unrelated, not sure if you saw Tanya added a section on UTIs and pyelonephritis to the CKD site)
 
Before I spent any money on an ultrasound. I would commence antibiotic therapy (something like Baytril) first to treat a likely infection, which even your vet acknowledges is likely. I would recheck bloodwork after the course of antibiotics. Then if things are still out of whack, I would proceed with further diagnostics. For all we know, this infection of unknown origin is something in the liver, causing the alkaline phosphatase to be elevated. It’s just prudent to proceed in this way.
 
When a cat has an infection, a lot of the bloodwork can be thrown off. Don’t lose heart. I’m sorry you are going through this! :bighug::bighug:
 
Gosh, I am sorry to keep adding comments, but if you don’t have a urinalysis (and a urine specific gravity measurement) on your kitty, then we can’t talk about kidney disease yet. And, dehydration brought on by illness will skew all those numbers as well —- so we are back together treating the infection again. That’s the first order of business!

Thank you, i'm going to call tomorrow and ask for a antibiotic. This has been happening for about 2 years now, same symptoms, throwing up, not eating, high white blood cell count. The other vet only used to treat the sympotms but this one i guess what the ultrasound because of the stomach issues i guess but the other vet always gave a anitbiotic

Yes. Specific gravity is a little low. Still need to treat the infection - as you already know. And as for kidney disease, there are lots of helpful treatments.
