? I did a curve! +11 108 now what?

OK with much stress poor Tinker got stuck like a pincushion today. His ears look terrible :-( but he got a lot of treats and was very brave :-)

So due to scheduling issues I missed the +10 and got a +11 reading of 108. Should I test him again in 45 minutes for possibly giving him a PM shot? Or just give the baby a break for the evening?

Also, going forward should I try giving him 1 unit once a day, or should I decrease the dose and still do it BID?

It's best to still get a PMPS test just in case his numbers go up or down. Great job getting the curve done today. Lantus works best when it is given BID. I don't know if you are following SLGS or TR. If you are following SLGS, he earned a reduction to 0.75 units. If you are following TR, you would hold the dose until he goes under 50.
 
Hi Tinker's mom. Did you end up shooting tonight? I agree with @carfurby that you would reduce if you choose to be SLGS but I also wonder if Tinker goes lower at night as well. For now, I would try your best to monitor and test as frequently as you're able to, especially a before bed test to see which direction Tinker is going before you sleep.
 
I am sorry to be dense, I really tried to read everything on the site. How do I decide which protocol I want to be on? I am dealing with personal health issues and don't have a lot of energy for testing right now. So is SLGS less effort?

I'll also summarize them here for you.

Start Low Go Slow
  • Hold doses for 7 days unless kitty earns a reduction (a number under 90 on human meters)
  • Do a curve at the end of the 7 days to decide whether to increase, hold, or decrease the dose. (Good job doing the curve!)
  • Always test before you give insulin and attempt to do spot checks here and there throughout the 7 days if possible.
Tight Regulation
  • Doses are held 6-10 cycles.
  • You need to test more frequently to catch kitty's nadir. It's common for people to do tests every cycle like: PS, +2, +6, +9, etc.
  • Reductions are earned when kitty goes under 50.
Basically, with SLGS, you test less every day and you hold doses longer. If you're going to follow SLGS, then definitely, reduce Tinker's dose to 0.75 units since he went under 90.

With any method, you do want to try your best to do spot checks and when you do, try to fill in the blanks. What I mean is, if for one cycle, you test at +2 and +6, try to test for maybe +3 and +7 in the next cycle. So that over time, you eventually have enough data to paint the whole picture.
 
It sounds like SLGS would be easier for you. It's what I use for Furball. You can reduce the dose when you go below 90 so you don't have to go as low to earn a reduction and it's better if you aren't able to test as much.
