How to go OTJ

Is there a specific process to go OTJ. We are using Lantus and are now down to 1 unit a day. Vet said to do this once a day, but I am waiting to speak to her about doing 1/2 unit 2x day. Is there a set up schedule on how long to do this, when to taper etc ? Thanks so much !

Hi Marie!! Are you home testing?

Your vet is wrong. Insulin, even Lantus, doesn't last 24 hours in a cat.

What we suggest here is to test several times a day.....if they can stay in normal numbers for 7 days (50-120 on a human meter with most of those numbers under 100), then you'd reduce the dose to .75....then test for another 7 days and if the numbers stay normal, reduce to .5....etc...etc.....all the way down to where you're only giving .1U twice a day before you try doing an OTJ trial. OR---if he drops below 50 at any time, you reduce the dose. We call it "earning a reduction"....we don't make reductions too easy to earn because we want it to stick!

You don't want to rush remission.....You want a good strong remission, not necessarily a fast one!

It would be really helpful if you'd set up our spreadsheet so we could see what kind of numbers you're getting so we can give you the best advice possible. If you look at others posts here, you'll see pretty much everybody has a link to their spreadsheet in their "signature" (the stuff that shows up under our comments) Here are the Instructions on getting the FDMB spreadsheet

We are really struggling to home test. I think we’ve gotten blood 4 times. I ordered a different meter (needs less blood) that should deliver tomorrow- hopefully that will help us. We had all kinds of error messages with the Relion Premier:( and the cat isn’t cooperating !
