Titan's mom (Paula) here. Titan was diagnosed in June with diabetes and recently we received the
acromegaly diagnosis. We are up to 4.5 u of Lantus twice a day and still have most days in 500/600 BG range. My once muscular guy is skin and bones and always hungry. We are on low carb food. He gets
a 5.5 oz can of Wellness Core chicken a day. We alternate days with Purina DM per vet's suggestion. After poke/shot treats are Fancy Feast salmon or chicken classic pate. I'm boiling a chicken breast for him today. Should I limit his food intake to a definite amount or can I feed more boiled breast or Fancy Feast to keep him satisfied? Poor Titan says he's starving.

Do not limit food intake. Feed enough to maintain weight (if possible).
Increase does in at least 1 unit increments.
Near the end my aero MurrFee was on about 50 units insulin twice daily. I used combination of Levemir and N since N is less expensive and some say Lantus stings at high dose.
The internist who saw MurrFee agreed with using N. Some aero caretakers used a combination of R and Levemir/Lantus

If he's hungry, feed him. Diabetic cats whose blood glucose is not yet controlled cannot properly utilize nutrients from their food, and are, literally, starving. Once his BG is better controlled he won't be hungry all the time. :)

Use your scale to figure out how much to feed him. Does the vet have a suggestion for an ideal weight for him? Weigh once a week to see if he's going in the direction you want. I would feed higher calorie foods if you want to add some weight on. There is nothing magic about the DM. The Wellness Core is higher calorie per can so a good choice. Chicken breast is a good very low carb option.

Acromegalic cats are often "starving". The acro tumour puts out excess growth hormones. Think about the appetite of a growing teenage boy, always wanting to eat. You need to feed them because they are growing. As Titan gets better regulated, he will be less "starved" but likely almost always have a very good appetite. We got very good at keeping the kitchen counter clean. If not, Neko cleaned it for us.

Neko and Titan must be kindred spirits. Titan has lost weight thru this ordeal especially early on. 15.5 lbs down to 11ish now and holding. That's very thin for him so we are hoping he can add on eventually. He has turned into a "counter cruiser" , never did that before.

Thanks everybody. We will start weighing him. Not sure I'm ready to know exactly how skinny he has gotten.
I will continue to feed him Wellness Core and prepare his chicken breast. I will do anything that keeps him
fighting.

My MurrFee used to jump onto the counter and learned how to open the cabinet doors and find something to eat before I locked the cabinet doors.
One of my Bengals does this, makes me crazy, lol!

(This is not her, but that's exactly what she does)
