How long does PZI last?

I got Ruby's bottle of PZI on December 23rd. There was one time when I forgot to put it back in the fridge and left it on the counter for a few hours but the vet said to not worry about it.
I try to not over mix it before each shot.

Right now I've only used about 1/3 of the bottle but I've noticed it doesn't seem to have the ghosty looking floaty thing when I first take it out of the fridge. It seems more cloudy overall now. With the swings Ru's been riding lately do I need to worry about the insulin having been overmixed or gone off some how? When do I know its time to get a new bottle?
 
There should be a expiration date posted on the side & the bottom of the box.

I left Baxters bottle out for almost 4 hours once. I was a little freaked out thinking...great there just went $130. It seems to be good still. It still gets cloudy.
 
I don't have a box, they put it in a pill bottle.

When Ru was first diagnosed, we transferred her from her regular vet to spend one night at the 24 hour clinic (it was the night before Christmas Eve). The 24 clinic put a piece of tape around the PZI bottle itself with Ru's name on it and it covered the original label. I'm almost out of syringes anyway so I'll ask the vet when I talk to them.
 
Maybe this will help... Baxter was DX Nov 12. His bottle expires 1/13.
So his bottle last a little over 1 yr.
