High units of insulin?

Just curious, but I've noticed that some people are giving their cats several units of insulin a day, into the double digits, when most commonly I see instruction for 1 or 2 units, 2x a day. Why are some cats given SO MANY units a day? Are they being fed high-carb food still? Is their BG just that out of control?

1/2 to 4-5 units twice daily seems to be the norm. It is dependent upon the cat and somewhat on the diet
Some conditions such as acromegaly require a high dose. My MurrFee was on about 50 units twice daily.
If a dose higher than about 5-6 units is required then many vets say one should checked for high-dose conditions like acromegaly.
 
Not all comorbidity, specific high dose conditions - acromegaly, insulin auto-antibodies, and Cushings disease are 3 conditions that can skyrocket the glucose levels.
