High Number at +5 Fur Shot Again?

I am not sure why that I am getting such high numbers at +5. I don't think it is a fur shot. I used a new shaved patch to give injection. Did not feel any on the fur?
 
Seems recently that you are not getting much of a drop in BG at all.
Does the insulin seem OK? You are gently rolling the vial before you withdraw the insulin from the vial right?
 
yes, I roll the bottle between my palms. This is about the last fifth in the bottle. I have been using it since June 12.
 
I will go to the vet and get a new bottle tomorrow. Hopefully they have it.

I am so upset. Thank you for your help.
