Help!

My vet recommended not to give my cat insulin if his BG is below 250. His BG has been between 151-175 and he has not been on insulin since 11-25-13. But now his appetite has suddenly decreased along with his activity and is showing a high amount of ketones but his BG is at 154. Any idea what is happening? The vet said to just keep monitoring him but I hate seeing him like this.
 
It is unusual to have ketones with lower numbers, but not impossible. If he has an infection and is not eating, it is a possibility. If he were mine, I'd take him to the ER VET and get him checked out. Trust your gut.

The reason I asked about the ketone test is that it is sometimes hard to be sure about the colors on the strips, but it should be accurate at the vet.
 
My vet did blood work and took a urine sample on Wednesday. Because Thursday was Thanksgiving I didn't hear back till Saturday morning. She stated everything was normal but he showed a high amount of ketones and wasn't sure why because of his BG readings being ok. She told me to buy the strips and test him over the weekend. If they were high, then i needed to bring him back in on Monday because she said there might be something else going on. I was only able to get two readings as he barely used the bathroom. Yesterday's results came back moderate and this afternoon it said high. Right now we decided to try and feed him raw chicken and he seems to like that a lot. So he is at least getting some food as opposed to just a taste.
 
My J.D. is a DKA survivor. He was in critical condition when I took him to the ER.
With high ketones, I would take him to the ER. DKA can happen fast. Signs are lethargy, lack of appetite, vomiting, etc.
 
I was hoping that I didn't have to take him to the ER but since ketones can be deadly if not treated right away i'll just take him. Since being diagnosed last July at the age of 5, this is his first time being off insulin. We had high hopes that he was going into remission but I guess that's not the case. He has been off insulin since Monday and it wasn't until late Friday that he started to not feel good. Thanks again for helping.
 
Going to the vet is the best thing to do when ketones are over a Trace.

I am sending good and wise vet vines for your visit and to your kitty Louie I hope that with a little vet care and treatment that he will soon be home eating and playing and purring.
 
What blood glucose meter are you using? Some meters read too low i.e. the freestyle butterfly ones and the true meters - so he may be higher than you think.
