Help with the Spreadsheet

I've just been recording Gertrude's reading in a notebook. I would like to start recording on a spreadsheet on here. I've look at them and I'm very confused on how to record the information. Can anyone help me out?

AMPS - 'AM' (morning) Pre-shot test number
PMPS - 'PM' (evening) preshot test number
U - units given
The +1/+2/+3 etc is for the test number taken that number of hours since the shot was given. We're worldwide so saying 8am can be anywhere within a 24 hour period by time zone. Using the number of hours since shot lets everyone easily understand. There's coding that will automatically color the cell by the entered number.
