Hiya, hopefully someone is able to assist Charlie had a hypo just 7days ago and his numbers tonight are showing just on the low stage again.
I'm currently at the wait 20mins to retest him before feed etc but unsure of what's best to do if his levels are lower or on the good scale?

You don't have a dosing method written down but I would think you are following SLGS.
Also I am concerned you do not have many tests done during the cycles at all, So we really don't know how low the dose is taking Charlie..
Is there any reason why you can't get any tests done during the cycles? Optisulin dosing is based on the nadir or lowest point in the cycle so we really need to see some tests done during the cycles.

From the SLGS dosing method
Until you collect enough data to know how your cat will react, we suggest following the guidelines in the FDMB's FAQ Q4.4:
Q4.4. My cat's pre-shot level was way below the usual value. Should I give the injection?
A4.4. There's no hard and fast rule, but if you don't have data on how your cat responds to insulin, here are some general guidelines.

  • Below 150 mg/dl (8.3 mmol/L), don't give insulin.
  • Between 150 and 200 (8.3-11.1 mmol/L), you have three options:
    • a.) give nothing
    • b.) give a token dose (10-25% of the usual dose)
    • c.) feed as usual, test in a couple of hours, and make a decision based on that value
  • Above 200 (11.1 mmol/L) but below the cat's normal pre-shot value, a reduced dose might be wise.
  • In all cases, if you are reducing or eliminating insulin, it's wise to check for ketones in the urine.
  • Above the normal pre-shot value, give the usual dose, but if the pre-shot value is consistently elevated, it's a good idea to schedule a full glucose curve to see whether a change in dose or insulin is appropriate. In most cases, the target "peak" value should not be below 100 mg/dl (5.6 mmol/L), and for some cats it might be higher.
So I would skip the dose tonight providing Charlie has not had any ketones in his urine at diagnosis. Did the vet mention them to you?
 
SLGS?
I'm using needle to give insulin is that what that means? Sorry im learning as I go with all this as the vet is zero help =(

Ive done a full day testing few days ago should I be testing more days with +3 and +6 so to get more data on how he is going with the doses.
As for Ketones the vet didnt mention she only wanted a sample dropped in to check for infection but he isnt going more frequently.
Ill give him food and skip tonights dose and start again in the morning.

SLGS is Start Low Go Slow and is a dosing method you can follow using Glargine. It has rules and guidelines you can follow which really help a ton.
 
SLGS is a dosing method. Nothing to do with using needles or how you give the dose.
Here are the two dosing methods for you to have a look at. You are not testing enough to do the tight regulation, so I think the start low go slow doing method is better for you at the moment. Dosing methods
Once you have a look and decide can you put either TR or SLGS into your signature and the top of the spreadsheet so we can see please Ally?
In regards to testing if you could get one test in every cycle, it will give us a much better idea of what is happening with Charlie and the dose. DO you think you could manage that?
 
Ok I’ll have a look at those but yes I think for now it’s the SLGS.

Try and get a test in sometime between +4 and +7 each cycle if possible. If that test is quite low, I would get another test an hour later to see if the BG has stopped dropping. The information during the cycles is very important so we can see how the dose is affecting the BGs and if it is enough or too much insulin.
Make sure he is eating well.
 
Ok I’ll definitely add that testing to our day :) trying to hold onto hope we will get it sorted and be on track.

I’m going to buy him a mixture of new low carb options tomorrow so hopefully he will enjoy something new and start eating more

You will definitely get on track if you follow one of the dosing methods mentioned.
I would suggest you start posting over on the LBL page HERE daily for help and advice. Lots of experience there.
