Help is needed again for an old foster kitty I use to have

Help please if anyone is out there that has delat with this.
I have posted before about an ex-foster kitty of mine having some major medical problems such as being HyperT, in kidney failure (getting sub-q and meds) and then was dignosed diabetic.

The adopter has been doing FANTASTIC and is home testing. He has been very close in getting Thomas (the kitty) into almost remission and had his number in the 100s but now I get this message and I am so worried for him. Does anyone know what could be making his number sky rocket like this out of no where. He is not getting into food he should not have???


From the adopter: Thomas is on Lantus
I feed him 1/2 can of K/D mixed with 1/2 can of Fancy Feast, then when he's eaten at least half of his meal -- and preferably more than that -- I give him his insulin dose based on the BG reading. I'm mixing the contents of one Cosequin capsule with his breakfast (none at dinner), and he's getting 100 mL sub-q fluids every morning, and at a different site (between the shoulders) than where I inject the insulin (right flank, forward of his leg; weekly Darbepoetin injection goes in the left flank).


He seems to be reasonably happy and alert (although aggressive toward Misty, which is unusual), but I don't know why the BG is going up over the past few days.



Here are readings for the past two weeks (which is when I cut the dosage to 1/2 unit for BG readings between 150 and 200):

first thing I would do would be to check would be an infection of some sort, teeth or urinary tract inflammation or infection

Agree with Dian, check for infections, but if that is not the case have the T4 checked again. Hypert kitties need their meds regulated, the dose often changes over time. If the thyroid is revved up again things would be metabolized quicker. If T4 is still in check, is the insulin still good? Also keep in mind their was a full moon and some kitties get wonky numbers during a full moon. I believe this was called a super moon, even closer than usual.

Thanks guys. I will be sure to let the adopter know that he might want to check to see if Thomas has an infection going on somewhere.

Does he get any additional tests in during the cycle? It would be good to see more tests so we can see how low or high he is really going.

And why does he keep adjusting the dose at each cycle? 1/2 unit, 1 unit? You/he may want to read the stickies in the lantus forum to better understand how lantus works and consistency is key when it comes to dosing.

Latus is dosed based on nadir (mid-cycle) not preshot numbers so is there more data to know what the dose should be? Lantus also likes consistancy, changing numbers a lot based on preshot numbers is like starting over every shot. If he used to be consistantly at 1U and is now fluctuating between .5 and 1U that may be attributing to the rise in numbers.

Has he always gotten the K/D? That is not low carb so that could cause the numbers to rise.

Diabetic cats are at risk for URIs and dental issues more because of the sugars in their saliva and urine so ruling out infection and dental issues is always a good first step.
