Heff has been diabetic going on 2 years with numbers that have been difficult to regulate. He's a very large overweight cat who was diagnosed with HCM in December. This weekend his numbers are sky high without an explanation. What are some things I need to look for & ask?

A sudden increase in glucose may point to an acute problem such as pancreatitis or infection.

The characteristics and tests to do are in the High dose forum. Here is the link for more info.

As you just increased the dose to 4 units, you may be experiencing New dose wonkiness, a phenomenon of paradoxically higher glucose with an insulin increase.

This post here probably has the most current information here on acromegaly. To me, Heff's SS does not have the look of an IAA kitty. IAA kitties have hard times getting to and continuing to see greens and get to higher doses than Heff has gotten to.

If you want to test for IAA and acromegaly, get your vet to do a blood draw to send to Michigan State University. The IGF-1 test is used to indicate acromegaly and here is the info on the IAA test. It's not a big additional cost if you are already getting and shipping blood for the IGF-1 test.

As for sudden changes in numbers, has the vet done a blood draw recently? Taken a look at his teeth? Infections are a common reason for sudden higher BG's.
