So we are giving him .5 every 12 hours, and when Gabe got home and tested him at 5pm (his shot is supposed to be at 530 AM and PM) his level was only 134. He called and asked so I said to just test again in an hour after food. He retested and Boots dropped to 120...he tested AGAIN at 830 pm and got 204. Then tested AGAIN at 930 and got 235...I am REALLY confused. And I don't want to give him a shot when he's that low because I don't want something terrible to happen while Im sleeping. Can anyone shed some light on this for me? Thanks y'all..his last shot was at 530 this morning, so it's been well over 12 hours, and Ive never gotten numbers like this for a pre-shot test..

That's normal. Cats do this.. The pancreas may be healing a little and bringing him more into normal numbers.

IF you were using a better insulin this would happen more and more often and you would start shooting normal doses at these numbers.. Shoot low to stay low. And perhaps boots would go into remission. But we have to be more careful since you are using such a harsh insulin.

Anyway He is rising now so you are probably ok to shoot but I would only give 0.25 since he still isn't that high. He will probbaly continue to rise now but get a test in two hours after the shot to make sure he isn't going to go too low tonite. Or you can skip and shoot tomorrow at normal time to get back on schedule.

Note that you'll need to eyeball 0.25 units as syringes don't mark that fiinely.

I think I will just skip it because we tested again at 11 pm and he dropped back down to 189. I'm so confused. I'm making sure to note all of this, (Im keeping a much more detailed written log as well as using the ss) and we will just test again in the morning and shoot then.

How is he doing on the 5 Ps (aka the Whole Cat Report)
- Peeing & thirst
- Pooping & eating
- Playing/activity level
- Purring/contentment
- Preening/grooming

He appears to be fine...I know he pooped this morning, the litter in the area of the box he uses is wet (though he has still been peeing on the carpet and Im starting to think that it's now more of a behavioral prob as opposed to medical) he is back to chasing my other cat around, he was terrorizing her this evening after I got home) still purring and his coat looks better than it has in months. Still dandruff-y but less of it, and it's shiny again and he's cleaning himself. However...he just started vomiting a couple hours ago. He did it once around 11 so I didn't think much of it and just about 30 seconds ago. It's not really bad though.

Make sure there is fresh cool water and leave him be tonight.
If he won't eat or can't eat in the am without vomiting, I think I'd go to the vet.

okay. He is eating fine, the vomit wasn't much of anything, but I am going to keep an eye on it, leave him some food by his water before I go to bed, and check for any vomiting when I wake up.

Well Boots is still fine. Eating well, keeping it down, drinking, he slept with me last night and purred himself (and me) to sleep. BGL was back up at 5am around 500 so he got his AM shot. I will be making his vet appt today for next week to get him on a better insulin.

Me too. And I just made his vet appt for weds, so by the end of next week he will be off this insulin and on lantus.

Have you started reading the Lantus sticky notes at the top of the tight regulation forum?

Also, what does Boots weigh and what should he weigh?
The starting dose is calculated as:
lean weight in kg * 0.25
where:
lean weight = either his current weight, or his ideal weight, whichever is lower
kg = pounds divided by 2.2

No, but I guess I should start reading up now! Boots weighs a little over 11 pounds (3 weeks ago at his diagnosis appt) but six months ago he was very overweight at 17 pounds. The vet didn't tell me what he should weigh, just that he was under weight. I have a lot of confidence in this vet..from talking to the tech over the phone it's a lot like talking to one of you folks, they believe the same things and even emailed me a list almost identical to the one y'all have here. Also told me to bring my home testing kit to save me the money of having them do a glucose test! I'm sure that this doctor will be more thorough with me about what is going on and what needs to happen, including weight gain/loss.

Some more homework for you Jade.

You might want to start calling around to find a pharmacy that will sell the individual Lantus Solostar pens. Not all will sell them singly.

11 / 2.2 = 5 kg
5 x 0.25 = 1.25 units.

You will need to eyeball this as it is between the half unit markings on syringes that have these.

I did that already, when I thought changing the scrip from vial to pen would be simple. The CVS down the street from my house said they'd sell to me by the pen.
I will share my vet experience with you when I get home :) 1.25 is what I will give him on the lantus?
