Hi. :)
Doing the switch from PZI to Lantus and I know it may seem like a silly question but I have 100 million reasons to not believe my vet, and I know it says on the box, but I am uncomfortable with not giving her insulin twice a day... ( nailbite_smile ) I cant find this info in the areas were there is directions to giving insulin so I thought I would ask. Thank you.

I've been looking into 1 per day myself as this may become a necessity at some point for me. The paper linked by Sienne is terrific ammo for the 2/day home testing approach, and here's some other reading,

A study on healthy cats on the effect of Lantus 1 per day vs 2 per day:

The results were pretty close which gives some promise to the 1/day hopefuls, but the authors state "We predict that better glycaemic control and reduced periods of hyperglycaemia would be achieved in a greater proportion of diabetic cats if glargine is administered twice rather than once daily." They do go on to say "A study involving diabetic cats is required to compare these dosing regimens, and to fully evaluate the potential of glargine for treatment of diabetes mellitus in cats."

Here's one comparing Lantus 1/day vs Lente 2/day in diabetic cats:

It found the two approaches worked similarly. They did Lantus 1/day as this would be an approach clients are more likely to be able to carry out, and do concede that results may have been different if Lantus were given 2/day.

However, when Lantus 2/day is compared to Lente 2/day in diabetic cats in a different study, Lantus won handily (PZI was also compared here):

From the last two you'd expect 2/day Lantus to beat 1/day Lantus, but a controlled direct comparison in diabetic cats would be ideal. 2/day does seems to be by far the preferred method by the people doing the research, and it does seem to yield the best results. 1/day looks feasible if you have no other choice though. Since you are the one giving the shots not your vet, and you're fine with 2/day, the research so far seems to back you.

Hi, Everyone ~

When our cat Willie was diagnosed with steroid-induced diabetes in 2007, our vet had us doing once daily dosing with Lantus. As our vet had us keep increasing that dose, we nearly hypo'd our cat. Please be very careful if you're going with once daily dosing. Although that might work for some cats, it was dangerous for Willie. Thanks to members of FDMB for teaching us about twice a day dosing back then!

Brian, those are great links. Thanks.

Just a note on the Glargine vs. PZI article -- this study is outdated and tight regulation is no longer implemented in the manner outlined in the article. It also used PZI, which is no longer available thus making the comparison less than relevant to ProZinc.

Fair points, the dosing was based on curves done in clinic and definitely not the tight regulation of today. However I think it's still a reasonable comparison between Lantus and Lente both 2/day and combined with the Weaver paper it gives an, admitidly indirect and somewhat tenuous, way to compare Lantus 1/day vs 2/day in diabetic kitties. If there's a more direct comparison, I'd love to know about it.

Thank you so much for the replies!!! And the most helpful links! This is my third Vet I've used for her diabetes and I think the Vets up here in Spokane are just out of date... (I had other problems too but, none of that matters now. :)) She's been on PZI for a year now and making the switch to Lantus has been oh so stressful but I know Im not the only one thats been through it... since I found this web site. :) Thanks a million for being here! <3
