Get back into remission?

My cat Goose was diagnosed with diabetes a few years ago, and a strict feeding schedule of high protein / low card wet food was enough to bring him into remission and allow us to stop giving insulin (yay!). He has spent the past few months living with a family member due to my own pregnancy-related health issues. Now that baby and I are healthy again (and out of the hospital), Goose is back home, but after testing his blood sugar I realized that the family member he's been living with must not have been following his feeding schedule closely enough, because Goose has been testing in the low 300s.

My question is: Where should I go from here? Do I take him to the vet to discuss putting him back on insulin ASAP? Or should I get him back onto his strict feeding schedule and see if that brings the numbers back down? I'm worried about letting this go on for much longer, since I don't know how long this was going on. I really trusted this family member and thought she truly understood how vital his feeding schedule was, but when I asked her about it she admitted that she would sometimes give him a spoonful of her other cats' food or let him lick out their dishes when they were done. So it's possible his blood sugar has been in the 300s for two months, which really frightens me. Also, he's only been eating half his normal amount for the past two days, although he isn't showing any signs that he isn't feeling well.

First, check for ketones asap. That's the most critical thing.
If ketones, vet visit/hospital NOW.

If no ketones just get back on track with the feeding and keep testing. Possibly a vet visit to rule out any infection developed while glucose was high.

Do you still have any insulin? If not, you may need to get a pen and help his pancreas out.


...and I think she may owe you the cost of the insulin!
Plus the comment that by not following your instructions, she could have killed him as untreated diabetes is fatal.
 
that happened to my Tiggy where I accidentally started feeding him HC. I put him back on insulin and switched the food back to low carb. However I have had to increase the dose to almost twice what it was last time, and no sign of OTJ this time. But he is happy and healthy and thats still good!
