First at home glucose test was 184 before meal and haven't started insulin yet any recommendations

Atticus was diagnosed 3/11. At the vet his glucose levels were in mid 300's. Just checked his blood glucose for the first time at home and it was 184. Should I move forward with administering 1unit of Lantus as vet advised? Should I feed him and then check it? I'm not sure if there were ketones detected in his urine at the vet, but I'm terrified to send him into hypo so I'm wondering if I should monitor BG levels for a day or two more or move forward with the dose. Any help would be greatly appreciated.

For people who are brand new to testing and feline diabetes and posting on the Feline Health forum, we suggest skipping if under 200. Did you do a food change recently? If you haven't already, I'd buy some urinary ketones testing sticks (such as Ketostix), so you can test at home.
 
Thank you so much for that guidance Wendy. I did begin him on the prescription wet food started yesterday morning. I will look in to the ketones testing kit and purchase those tomorrow. I suppose I will skip it for tonight and check his bg again after he eats. I have been on this website all day and have read about how that between 150-200 you can wait, give a smaller dose or feed and recheck. But since I have never given him insulin before I figured the course of action would be different.
 
If he was eating higher carb dry food before hand, then changing food can make a huge difference in the amount of insulin he needs. I'd skip tonight and see what tomorrow brings. As was mentioned in your Feline Health post, you don't need to feed "prescription" wet food. The ingredients aren't anything special and many cats grow tired of the taste quickly. Any commercially available low carb wet or raw food will do.
 
Thank you Wendy. The vet gave us a prescription for antibiotics because he has gingivitis. I'm wondering if the antibiotics have been helping bring his BG levels down and if the gingivitis could be culprit for the increased BG. I will continue looking through the Feline Health post. Thank you again
 
Yes, the combination of dealing with any infection/inflammation (which can raise the BG) and a change in diet can help. Coupled with that, cats often read higher at the vet clinic than they do at home. Hopefully you'll be able to get Atticus a dental soon. Tooth problems contribute to higher numbers.
