My 14 year old Shadow has today been diagnosed with Diabetes. She currently weighs less than 3 kgs (part Maine Coon) and has simply wasted away in the last 2 months. She should weigh at least 4,5 kgs. Her muscles has also wasted away in the last few weeks. I Initially took her to the vet as she was vomitting and looking decidely unwell. They gave her an injection and sent her home with nausea medication. This was around 3 months ago. She progressively has just lost weight, and generally did not look well, so I took her back. They kept her in hospital for 2 days and was diagnosed diabetic. I have had to inject her with insulin since then, 2 x a day at exactly the same time every day. I was also told she had to be on hills dry diabetic cat food which she refused to eat. I tried the Royal Canin dry - no go either. I then managed to get Royal Canin wet food pouches for diabetic cats that she has been eating. Almost impossible to get right now in South Africa, due to Covid, so I have been phoning random vets around the country and couriering in a box or a few pouches, from here and there and everywhere. As you can imagine its very expensive and an ongoing nightmare to source. Shadow was initially on 2 units of Lantus Insulin, 2 x a day, but was looking so unwell I took her back. Enormous appetite, drinking vast amounts of water, seemed to be confused, unsteady on her feet. The vet reported her glucose was so high that it could not be measured. They kept her in, and could not get it down, so increased the dose to 3 units 2 x a day. Her kidney functions tested normal. Since Diagnosis, She has only got thinner, cant poop, her beautiful long thick fur is now scraggly, thin, and even falling out. On Monday, I had to rush her back, as she kept trying to poop but was collapsing as her back legs could not hold her up. She had to enemas to help to clear her colon, her glucose was tested 2 x on Monday, 2 x yesterday and all 4 times so high it cannot be measured. Still in hospital and still on a drip, early this morning my Vet informed me that the Glucose tested suddenly at 8, before she had even had her morning dose this morning. He said he is convinced that she has Brittle Diabetes, and while her glucose is at 8 cannot obviously have her insulin dose. He will be testing her again later. Update: He has just called to say that her glucose now has tested 16, (12 Noon) and they will test again before 5. He said more likely she will require 3 units again at 5pm if the glucose continues to climb. He fully expects it to climb. Right now I am feeling completely overwhelmed. Getting her diagnosis for normal Diabetes, only 3 weeks ago, was in itself just shocking, and this unstable Brittle diabetes is very overwhelming, as there seems to be almost no information about this. The information that is available all says the prognosis is very dismal, and ultimately swiftly leads to organ failure and death. I came across this page in my search, and hope I am at the right place. Any help or advice to assist my beautiful Shadow, would be greatly received...

I am sure we can help you in a number of ways.
I don’t think Shadow has Brittle diabetes. I think Shadow is dropping low and then bouncing up high again. If he was very high, then 8 then 16, all on the same dose…that is from bouncing. Bouncing can happen when the cat drops too low, too fast or lower than the cat is used to. The cat panics and dumps stored glucose and counter regulatory hormones into the system and the blood glucose shoots up high. Bounces can last from 1 to 6 cycles and there is nothing you can do but wait them out.
This is very common in diabetic cats. The only way we can see if he is doing this is to test frequently so we can see what is happening.
Im not sure the correct thing to do was increase the dose to 3 units if she had dropped to 8 in the vets surgery.
Cats get stressed at the vet and their blood glucose can be artificially elevated.

Are you hometesting the blood glucose ? That is the best way you can keep Shadow safe and find out what is the best dose for her.
You would need to buy a human meter from a pharmacy and some lancets (size 26 or 28 gauge) and some cotton balls. we can help you learn to test.

I would also recommend you set up a spreadsheet to put all the data into. Then we can help you dosing. We recommend going up in 1/4 unit increments so you don’t miss the best dose. If you have any trouble setting the spreadsheet up, let me know and someone will help you.

FOOD. There is no need to feed a prescription diet. Any low carb food is ok. I’m not sure what you have available in South Africa. Do you have fancy feast? Most canned food from the supermarket is ok as long as it doesn’t have gravy. We can probably tell you the carb content if you give us the other information on the can. Carbohydrate content of 10% or less is e-commerce dead for diabetic cats. Dry food is all high carb and not recommended.
Also diabetic cats who are not regulated can’t absorb all the nutrients in the food and will be hungry and can lose weight. So you may need to feed more especially if she is losing weight. Are you feeding a good meal before each dose and then giving a few snacks during both the am and the pm cycles?

I agree with Bron about the bouncing. The only way to find out is to home test – before each shot and a couple of times through each cycle, if manageable. We can help you with that.

I've found some low carb wet foods that look to be available in South Africa (waving from Canada) – Sheba, Weruva.
Go through these lists and see which low carb options are readily available.
 
Thank you so much for your assistance. Been trying to read everything I can that you have available on this site. Its a lot to take in. Certain Vet shops have Weruva, and only 1 close to me stocks little big paws. Weruva is only available in the pouches, and the same ones listed on your list. We do have Whiskers in the supermarkets, but not the pure delight one that is listed. Our supermarkets stock various Whiskers and Pampers in the pouches, gravy and jelly, but those you said should be avoided. The prescription food is either Hills or Royal Canin, both diabetic pouches are still out of stock in SA. Fancy feast in the little tins used to be widely available, but I have not seen it for a year, so not sure if it no longer gets imported or still due to Covid that is not available. We have not had the same ones that you have listed here, as I bought it from time to time, and none of those options listed are familar. I have bought Little Big Paws before, and my cats do love it. Its brand new, and as I said, only 1 Vet Shop that I am aware of in the South, stock this. It is however, more expensive than the Hills or the Royal Canin, but smells better and they will eat everything on their plates when I give it too them. None of the Australian or New Zealand food is available here. As far as Shadow goes, she is back at the Vet at 9 am this morning, still weighing in at just under 3 kgs. Still on 3 units of insulin, and the Vet spoke to me and said that he does not want to reduce the insulin at this stage. When she arrived there, on testing, her glucose was unreadable over 30, and right now, 3 hours later, its down to 20, but still nowhere near low enough according to my vet. He has not mentioned a home testing kit, but I intend to talk to him when I go and fetch her at 5 pm this afternoon. Home testing kits (for humans) are readily available online and in almost every pharmacy, so I can get one very easily. They are keeping her in to monitor her Glucose levels over the day again. I am really so very grateful that this forum exists and am trying to learn and take it all in so that I can do the best for my kitty and keep her comfortable, and feeling well. Her thirst and hunger was insatiable this weekend, and even though I have been feeding her little amounts every couple of hours, amounting to the right amount of food over the day, I have now had to remove the dry food that was always there for a snack. If I dont, she just keeps on eating until everything is finished.. If I poached her some white fish (Hake, similar to Cod), or Norwegian Salmon, and mixed it with some rice would that be ok? OR, should it be absolutely no rice? I do not know if she would eat this, but if she does, then I would be good to go on that. Should anything else be mixed with the fish?
 
Whiskas in Jelly are low in carbs and okay to feed, but Whiskas in Gravy are high carbs. Little Big paw is low carb.
You do not need to need any prescription food. The only low carb prescription food is the Purina ProPlan DM canned food - but it is expensive and a lot of cats don't like the taste of it.

If you can share the link to a local wbesite selling pet food, I could take a quick look and see if I can spot any diabetic friendly options.

Rice and vegetables are also a no-no since they are all too much carbs. I am not sure if you have any commercial raw food available in SA?

I would strongly suggest you check her blood sugar at home. Curves done at the vet can show high elevated levels on account of stress.
