So after yesterday http://www.felinediabetes.com/FDMB/...-60-5-54-6-51-7-79-9-120.182312/#post-2021637 when Fearless earned a reduction to .75u, I figured he'd bounce. He did, but only into the low 200s. This morning I had an appt I couldn't miss, but I figured starting out at 200 and with the reduced dosage of .75u that he would be fine for 3 hrs. I was wrong. He dropped to 33. He's fine now after gravy and cooked chicken, but here's my question: Reduce again to .5 unit only one cycle after the last reduction?
 
As Tanya said he has earned a reduction. Please continue testing since the gravy will wear off quickly and you want to make sure the numbers don't drop back down.

Another question about the depot. In that same post, I read the following:

Do you think the lower numbers this morning may be due to the depot not draining, even though Fearless did have higher numbers at PMPS last night and AMPS today? And, since I forgot to reduce last evening and basically shot through the bounce, should I expect that depot to affect BG readings over the next few days?

This has been a strange few days ...
 
The depot may be at play to some degree. However you hit 33 which is far too low to continue holding. Try the reduced dose for a few days...see if there is any bouncing from it first and if the numbers start to rise and stay up then you can consider going back up.
 
I'm going on vacation a week from tomorrow for 6 days so I hope by then that Fearless is at a point when he's on a consistent dose that keeps him at a consistent level. Then we can reduce it a little while we're gone since the sitter doesn't test.
 
See how Fearless is doing in 6 days time and then you can post for suggestions on what sort of reduced dose might work best while you are gone.

Hopefully he won't bounce too much from the low today.
 
Congratulations! I hope the new dose will work well. I don't think Fearless bounces so much anymore, which means the pancreas is healing and not panicking at the nice green numbers. :bighug::D

Sorry to just respond to you but, yes, absolutely. On 8/15, it looks to me like his PMPS was higher because he lost duration. If you had to feed a lot of high carb food or karo during the cycle to get the numbers up or you fed after nadir, than that would be a reason why he lost duration (there are other reasons but that would be first on my list for this specific cycle).

If it had been me, I would not have dropped his dose again so quickly from 0.75u to 0.5u. I think the 33 was likely the depot from the 1u dose so i would have dropped the dose to 0.75u and then waited to see what happened over the next few cycles. If, within four cycles of the reduction to 0.75u, he had dropped below 50 (not 40 as he is a newly diagnosed cat), I would have taken a reduction to 0.5u at that point.

Now that you’ve reduced him, let’s see what he does but there is the potential he will need to go back up to 0.75u.
 
I think you're right. We'll see how quickly he recovers. It's complicated because he's bouncing off the 33 too. I hate to waste several days waiting for him to clear the bounce then see what the lower dose does. He was doing so well before this. Does anyone know how long in the green and low blue it takes to have real healing effect on the pancreas? And if the pancreas starts healing and then the cat bounces so high and stays high for several days (on a reduced dose no less!) will all that progress be lost?

I understand but you don’t want to dose hop which can mess with the depot. I’d give it six cycles and if he doesn’t clear the bounce back into green nadirs, I’d increase to 0.75u.

ECID on how long it takes in normal numbers for beta cells to start to heal. It’s hard to say on whether progress is actually “lost” when they bounce. If it were me, i’d just focus on getting him to the correct dose under the TR protocol so that his BG is in the 50-120 range for most of the time. That’s when the healing occurs. The first step is to get them flatter and well regulated.
