Evening insulin

I just tested Joey with the accu check twice. It was 68 and 70. He has eaten very little today. He is suppose to get 3 units of humulin at 8:30PM..he had 3 units this AM at 8:30. That's my dilemma. Does he get his 3 units. The doctor has never called back all day. Thanks for any advice. I know you know your stuff on this site.
 
No, you would not shoot at numbers under 200 until you have a lot of data to know what his patterns are.

Are you at preshot time? If so, I would skip. If you have a couple hours, you can test again at +12 and see if he is over 200. With numbers that low far into the cycle, it would be wise to reduce your dose.

If you skip tonight, he may be high in the morning (24 hours since insulin shot). But don't shoot the 3 units again.
 
His assigned time is 8:30PM....Thats in slightly over 2 hours......He has eaten but very little. Maybe the equivalent of 1 small can all day. He actually is acting ok at the moment.
 
In some cats, you will get much lower numbers on Humulin if they are not eating well. So it is hard to tell whether that is part of why he is in the 60s or if he is really that low. I would feed him his favorite to see if you can get him to eat a little before his evening shot. (if he is high enough to give one)
 
With that low of drop, his humulin needs reduced. I would drop to 2.5u at least if not 2....WHEN the number goes up. BUT remember, Humulin is a 'one shot' insulin. When it begins rising, it should continue to rise as that means one of two things that you have to decide:

1) It's rising because the 'one shot' has done it's job. Takes another injection to create another 'cycle'
2) It's rising because you fed him a high carb food. It could go back down if it's early in the cycle

Both need shot for the next cycle. That was an awfully low end to that roller coaster for this insulin at this point.

It's truly up to what you feel comfortable doing.
