? Elevated magnesium (Copernicus)

His magnesium is elevated at 4.1 and the vet is blaming the Zobaline. She said supplements are unregulated and she's not familiar with that one. I only started the b12 because he is weak and fatigued. It didn't cause the weakness and fatigue. Sodium 148 and potassium 5.5. Any issues with Zobaline? What else would cause elevated magnesium? I was told he didn't have CKD last blood draw, but now I don't know.

What other supplements is Copernicus on? Any of them contain magnesium?

I haven't seen Cushings kitties comment about high magnesium, but I did see one reference say it could be caused by Addisons, as well as a number of other things.

I'm frustrated with vets. I just found out his IV wasn't started until 2am and they drew another CBC at 6am. The cat is already anemic and that wasn't the plan last night. He was supposed to be on fluids for 12-24 hours before his next blood draw. I can't even talk to a doctor. I'm talking to them through students and feeders. No vet yet has mentioned Addisons either.

He has a 5mg Pepcid, could it be the compounded Cobergoline? No other supplements.
 
Pepcid contains magnesium hydroxide. In tablet form, cabergoline contains magnesium stearate. But I don't know what is in the compounded form, that's a good question for the compounder's pharmacist.

His phosphorus is too high, it will make him feel icky. Maybe he needs to eat a lower phosphorus food.
 
The Tiki cat silver chicken and the Weruva Wx both are labeled low phos. I guess it could just be marketing but I thought those were trusted foods.
 
I agree about the phosphorus being too high.

Calcium x phosphorus should be less than 65(ish) per my IM vets; higher than that, the internal organs can start to mineralize. I had to put Tubby (and some of my other kidney cats) on phosphorus binders. Low phosphorus food helped some of them, but Tubby had other medical issues that prevented me from changing foods so phosphorus binders it was.

I recall we talked about this last time and I asked the vets and they didn't want to start him. The IM at vet school never even mention the phosphorous on the phone yesterday or today. She just kept talking about the b12 and magnesium. He's staying another night bc magnesium hadn't come down.

I questioned the cabergoline and I was dismissed because they said they compounded it there. But he has been on it about 14 days so it could be the cause but they are tunneled on this b12. I bought Zobaline from the website and not from Amazon.
 
So sorry you’re dealing with this, Krista. Maybe this would be helpful for your vet. It’s the 2023 IRIS treatment guidelines. They talk about keeping phosphorus certainly under 6 mg/dl, and ideally under 5 mg/dl using a binder.

I’d be curious what his kidney markers were last time you had labs… because I would imagine things would have been gradual to get to this point. Unless there’s something else that would spike his phosphorus/BUN, not sure.

Let me pull all his labs this week and get organized. That would be good to see, but I think his labs always look good after getting out of the hospital and then he ends right back in for something. He's really grumpy. He's getting tired of all this. He needs iron, he needs binders, he needs subq, he needs b12, maybe I need to find a functional vet if those even exist here. The vet school said fluids is old school thinking and they would place an e tube or g tube ? for fluids at home.

That feels odd to me… if he needs fluids, the IRIS document talks about subcutaneous fluids as an option. It does also talk about feeding tubes being an option for late-stage cats. If he needs them I’d be looking at subq to start unless you can’t do it that way for some reason. But see what JL and the others think. Ugh it’s all so complicated, hang in there :bighug:
