Ebby +5 56 Lantus started yesterday

I think I have this under control, but haven't dealt with these numbers yet.

New lantus insulin started yesterday.

+ 5 is 56.

I do not know how she responds to this low number so fed her 1 teaspoon medium carb food. Should I be doing the high carb? Will retest in 15.
 
wow, good numbers! I know that must be scary after steady consistent ProZinc numbers. Watching you and Ebby with great interest! :mrgreen: :mrgreen:
 
Yeah, to see a 56 shocked me. I probably shouldn't have fed the medium carb food....maybe just low carb, because now she is at 246 at +7.
 
Better to feed whatever to get the numbers up if you are not sure how she will react.

For future reference, you only need to feed the LC (low carb) when numbers are in the 50's. About 1 teaspoon should do it. If the numbers keep dropping, you would have wanted to feed more LC until you hit the low 40's then start with some MC. A bit of Karo on the gums if below 40.

If it had been earlier in the cycle, around +2 or +3, maybe a bit of MC (medium carb). And monitor.

We want to keep her surfing in those nice greens as long as we can.

You may need to drop the dose of Lantus. Let me look at the SS and get back to you.

ETA: Yes, since only first day on Lantus and depot has not filled yet, I think you want to drop the dose to 0.5U maximum, maybe even 0.25U. Please, get a +2. It can help tell us which direction she may be headed. Early days with the Lantus and the more data you can gather the better. +3, +6, +9 are suggested for the first couple of days on Lantus. Tests in the +4 to +7 range would be good to help pin down her new nadir with the new insulin. Some cats nadir early, some nadir later. +6 is only an average. ECID.
 
Thanks Deb for the info. I think this was a good trial run in dealing with low numbers. (Especially being 4:30 in the morning.)

I looked up the "MC" food I fed her to see the carb content again, thinking maybe it was accidentally a high carb. It was Merricks Surf and Turf which is actually low carb. (3 grams carb) So maybe by accident I actually did the right thing!

I doubt this food would have spiked her into the 200's within two hours....do you think that is just a bounce from her seeing those greens?

I think a dose reduction is good, especially since she got pretty low on the first day of Lantus. I'm thinking .5 U since I will be around to monitor. I will test at +2 and try to do mini curves. Should I post these in Lantus?

Since you are pretty experienced you may well want to post in the lantus tight regulation forum where they can help advise better on dosing. I do think today is a bounce and a reduction is needed since you got such a low number even though the lantus depot hadnt built up.

I wonder though whether there was some kind of weird overlap between the PZI and Lantus so you may end up having to increase the dose again but since we dont know that for a fact we are best to play it safe...
 
I see you have decided to post in the ISG Lantus TR forum. Good group of people there and they will help you a lot.
