Double dose of Lantus this morning - :(

Arggh, I am so frustrated and I know that I generally do not post here very often, but I am just so.... frustrated and worried.

My cat, Dodo (Little Boy) just had a dental done last Friday (he had 5 teeth removed and a mass from his leg). Over the past two nights, he has been throwing up his food so I took him back to our speciality vet this morning (who also did the surgeries). We are not sure if perhaps it is just the antibiotic (climadycin) or if he has had some unexpected reactions to the anesthesia, or if his pancreatitis is flaring. He has a full docket of tests and things today-- more in depth blood chemistry panel, ultrasound for pancreas, and we will be doing a chest x ray since the mass found on his thigh was a leiomyosarcoma and although it was well encapsulated and small I do want the chest xray just to make sure that nothing has spread.

Apparently when I got home from the vet and told my mom what was going on-- she said something about he was squirmy with his insulin this morning and she was not sure if she got all 2 units in of Lantus. I looked at her and said what are you talking about? I told you this morning I had given him his insulin right after I popped the antibiotic in his mouth. I asked her if she was sure that she had given him insulin-- she was. :( Now we are looking at a potential extra 2 units of Lantus. I called the vet immediately and the nurse said she would be telling my vet.

Now I am all worried about him at the vet-- he is already on an empty stomach and not feeling great. :(
 
Ok you need to test him frequently to see what the BG's are doing.

You also need to get food into him to keep him from dropping and hypo'ing.

Do you have any high carb gravy foods? You can just feed him the gravy, spoon, syringe it if necessary. Do you have karo or maple syrup or any simple sugar products? You may need this too to get him higher.

If he drops really low below 50 and you can't get any food into him - taking him to the vet so they can administer glucose drip may be the best option.

What are his bg's? I also recommend that you post on Lantus board and add 911 to your subject line, so that you can get eyes on your post.
 
Hi Hillary it is kind of out of my hands for at least the next few hours. He is at the vet in the ICU department (ICU just because he was being monitored for appetite and getting some fluids into him today since he has been eating poorly).

I told the vet nurse - although I have to admit my OCD control freak side of me wants them to be testing him continuously.

I just hate that he is not home so I can keep an eye on him-- although to be fair I couldn't get any food in him to stay down until we went to the vet this morning.
 
If he is at the vet, then please ask them to test him and to give him glucose drip just in case - you don't want him to hypo in there.
 
Hi--

His AM numbers were 331.

He has been running a tad higher than normal since the mouth surgery and the antibiotics.

I didn't get another test in this morning since as soon as he ate breakfast he puked up all of the antibiotic and food. I took him directly to the vet since this is not normal for him.

My vet is supposed to call and give me an update on him in another 1.5 hour.

Poor guy he needed this like a hole in the head on top of everything else going on with him this morning.
 
I would call and talk with a tech now, and not wait 1.5 hours. to be sure that they understand that he received double his insulin dose. And you are concerned, would they please test and even give him glucose drip because he isn't eating.
 
Vet called-- said DoDo was doing fine. They are closely watching him and have him on fluids.

The plot thickens-- his kidneys and liver are normal so that wipes out the concern for the anesthesia reaction.

They are doing an ultrasound to check on his pancreas -- which if it has flared up again I don't know what the next steps will be. Last time when he had a bad flare up we had to put in a feeding tube when he came home from that and the DKA.
 
Just a final update. Little Boy Dodo is home now. He came home yesterday afternoon.

They ended up not using a dextrose drip and just did close monitoring on his blood sugars. They were able to get his blood sugars back to a solid number with food alone (they used combo of gravy filled fancy feast and hills ad). We stopped the antibiotic climadycin and he is on cerenia to help with his nausea. He was also given fluid support to help him through any of the tummy attacks from the pancreatitis. His appetite was back to normal tonight thank goodness.
 
that's great - for future reference, while you too can bring his bg levels up using gravy and high carb foods, given that he wasn't eating, it was important to take him to the vet. so please don't question that decision.

It is good to be prepared for this sort of thing, very low bg's by having a hypo kit ready - are you familiar with this and do you have one? also buy FF gravy lover's as a good high carb food to put in the kit. and definitely keep it separate from your regular foods, just so it doesn't get mixed up - even label it high carb just in case.

Many antibiotics cause GI upset.

If he needs one again, about 2 hours later offer a probiotic like Forti Flora to replenish the good bacteria knocked out by the antibiotic.
