Dosing Advice!

I've been treating my guy for 6 weeks on PZI gradually increasing dosage at my Vets advice and testing now I've done 2 curves in the past week at the 4u level!

Vet wants to go to 5u and do another curve! I've done 2 curves in the past week and am doing a mid cycle test when I can. His pre shot numbers are still very high but I seem to be seeing inconsistent numbers mid cycle and his 2 curves were conflicting. Vet wants to hold off on food during a curve cycle unless I get a hypo number during the curve cycle. Is it preferable to only feed ate the shot times in this case to see the numbers for 12 hours without the effect of food on the 2 hr intervals?

Welcome! Your cat is very reactive to the insulin and this causes wide fluctuations from pre shot to mid cycle that we'd call bouncing. He also bounces to high and flat numbers. You're still very early in his treatment.

Vet wants to hold off on food during a curve cycle unless I get a hypo number during the curve cycle. Is it preferable to only feed ate the shot times in this case to see the numbers for 12 hours without the effect of food on the 2 hr intervals?

When doing a curve at home you test every 2 hours from the AM pre shot to the PM pre shot. Most cats do better with at least one meal between AM and PM. It's possible to time it so it's given right after your mu=id day curve test. By the next test two hours later there won't be a large food effect. The goal of a curve is to see the action profile of a dose over a 12 hour cycle. It makes sense to me that it should capture a normal cycle where food is given at least once through the cycle.

I suggest you continue collecting BG data at 4 u mid cycle somewhere between +4 and +7 any day you're at home and aim to get one before bed test every day. You need to collect more dats at these times to judge how to proceed.

We never recommend whole unit dose changes because it's too easy to zoom past a good dose. Many vets want increases that are too large.

Thanks in my case due to work schedule I'm at a 1:30-2 am and pm schedule! I've been doing my curve when I'm home in the daytime. As far as nighttime I've been getting up at 7-8 am to feed him mid cycle. I've been feeding him every few hours though during the day/evening!

You have to work within the constraints of your schedule. We recommend getting a scattering of single tests here and there in day and night cycles. Over time these build a better picture of how he’s responding. Think of it as a jigsaw puzzle that you’re assembling. A curve is of limited usefulness, rather like filling in only one edge of that jigsaw puzzle.
