DKA and UTI this weekend--continued HI amps values

Kitty had DKA and a UTI this weekend and is still in the vet. She is receiving 3 units of PZI twice a day--but her amps number is above 500 everyday this week.

This morning she was HI at 8am and dropped to 418 at +4. The Ketones are gone and the vet is treating the UTI with Zeniquin/antibiotic.

Kitty is pretty laid back and as never seemed very stressed at the vets. So, outside of stress and infection is there anything that would keep her amps number so elevated?

Her amps had been in the 350 range prior to the weekend crash.

Just getting anxious and looking suggestions.

I have been reading about Cushing's disease and wondering if that could be going on.

Hopefully it is just the infection and once it is under better control, the value will come down. I want to bring her home, I just don't want to remove her prematurely if there is something else going on.
 
If all the glucose values are in your spreadsheet - it looks like you are still only giving insulin one time a day. None of the studies on the newer insulins (prozinc, lantus, levemir) call for dosing once a day, especially with the high numbers you are seeing. Cats metabolize insulin quickly and almost always need insulin twice a day. You will continue to see values at this level until you switch to twice a day dosing because kitty is essentially going 12 hours a day without the help of the prozinc insulin. Please ask your vet about going to twice a day dosing!!!!!!
 
They started twice daily dosing this week (3 units twice a day). But since she has been in the vet and they are checking her numbers--I don't have them to update the sheet.
 
does she display any of the physical signs of cushings? if not, then don't worry about it just yet. It could be that the massive increase in insulin and/or stress has something to do with the numbers. While very wary of ketones, I'm very worried that she may be getting too much insulin.
 
If your kitty is still hospitalized and being treated for DKA it is way too early to look for insulin resistance issues.

Every value is skewed when a cat has DKA...and it can take some time once kitty is home for the numbers to fall into a better range. Each cat is different, of course, but it usually took Layla a month or two after recovery to get back into normal numbers.

is she still getting IV fluids?? I would hope it is not dextrose. are the just treating her with PZI and no fast acting N or R to bring numbers down. just asking, do not know what the norm would be and like said ECID and I guess that would also go along with EVID (every vet is diffrent)
