Diabetic or?

My 4 year old girl just had a UA with 2+ glucose and a bg reading of 212 but this was right after a dental cleaning. All previous blood work she's never had elevated glucose. Her urine specific gravity was also low and diluted. She only received 4ml of IV fluids so the Dr highly doubts it's from the fluids. She's concerned she has diabetes. She eats 1/8 cup of Dr elseys clean protein kibble and 1/2 to 1 can of fancy feast gravies and 1/2 can fancy feast naturals per day. All of June she was on Purina EN for the dry instead of Dr elseys and I believe it caused her horrible gi distress with 4 big hairballs which is abnormal for her and why we switched back to elseys.

In May everything looked great glucose wise and urine was very well concentrated and had crystals in the urine and only reason we checked it today because we wanted to make sure the crystals weren't accurate and were from basically sitting about 24 hours before processing at the lab so we ran urine right away today. She has had multiple urinalysis done and blood work and never showed this. We added on a fructosamine check today.

I want to add she has always been hit or miss with eating. I adopted her at about 1.5 years old and she previously was a wanna be feral I call her. She only bonded to me and she has a strong deep distrusting nature of people. She usually does her eating over night as I have two sure feed microchip feeders because during the day she will sometimes only eat one meal. At night I put a serving of wet in one feeder and hairball treats and the 1/8 cup Dr elseys in the other. I'd say most days she eats everything but some days she doesn't eat any of the wet. She has good and bad days with eating ever since I got her. I even took her to an internal medicine specialist last year and they suspected IBD but she had healthy poops and the vomiting is from hairballs

The fructosamine test should tell you if this was a one time high glucose or a trend. I had a semi feral who once tested over 220 at an emergency vet clinic (possibly urinary blockage with crystals), and in the 50's when I tested him at home the next day.

How often are the hairballs? Egg yolk powder might help, read this article: How Best to Manage Hairballs

Thank you. The hairballs usually aren't once a month but sometimes comes in waves like when she was on the purina EN kibble she got 3 massive ones that whole month she was on it so I changed her back to Dr elseys and hasn't had one this month. I use the egg yolk lecithin powder because she likes the taste. Maybe half a capsule a day and adored beast gut soothe I put in water and syringe it in her mouth daily

I wanted to add her fructosamine came back at 377. I'm not sure how far back that tests but June 27th I stopped giving her the purina EN naturals dry and changed back to Dr elseys dry for her night time meal and the fructosamine was done July 7th. The doctor is emailing an internal medicine specialist because she's confused/ concerned and never seen a diabetic cat not on steroids this young before. She isn't saying she's diabetic but the fructosamine and 2+ glucose in urine concerned her

More details on the fructosamine test here: Reference Ranges for Fructosamine Test. You would need a couple weeks on the lower carb dry food to see best fructosamine results.

We've seen a few younger diabetic cats here. This is the story of one of them: NEW MEMBER Hi, Anyone here with a Juvenile Diabetic or a Complex Case? We've also seen one diagnosed at 10 weeks old! And a heartwarming story of one diagnosed at 6 months: 9/7/2023 - After 10 Years, Michelangelo is OTJ!
Thank you very much. I stopped giving her the purina dental treats too as they're mostly corn meal. I will recheck her UA and fructosamine in 1 month is the plan. Ugh. This makes me sick to think about. I was hoping for a break with medically complex cats and I guess I had a few years after my Morton but it doesn't seem long enough. The only high carb thing she gets is half to a full can of gravy lovers fancy feast a day and I really don't see how it could be THAT bad considering most people feed all dry only with like 30 percent carbs even the wet food vets recommend can be 30 something percent!

Some cats are just very carb sensitive so something even just over the recommended 10% carbs can cause blood glucose levels to become elevated. Can you feed only foods under 10% carbs? I think some of the Weruva / Soulistic foods are in gravy or sauce and are lower in carbs than FF. Tiki Cat might be another one.
