Hello-

In another thread here, I was told that my cat showed no sign of kidney disease based on his blood test results, but the vet told me my cat has kidney disease. My cat's 2 most recent tests: creatinine 2.0 and 2.4. The IDEXX result sheet lists 0.8-2.4 as the normal range for creatinine.
But IRIS (http://www.iris-kidney.com/pdf/IRIS_Staging_of_CKD_modified_2019.pdf) has this level of creatinine as stage 2 kidney disease: 1.6-2.8.

Re. SDMA, my cat's levels were 8 and 11, and the IDEXX normal range is 0-14.

Could you please clarify?

Karen I've just looked back at the labs on the SS and you haven't entered the USG on either of the urinalysis. It is on both the sheets you originally posted. Could you have a look at the numbers and enter them please. The one from October is 1015 but the one from January which I thought was 1033 (its blurry when enlarged) could be 1013 which is not as good as 1033 and does indicate some issues.

You asked me some questions in the previous post which I will answer here. I am no expert though, ......
I said in the previous thread that I could not see any real reason why Maxi would have early kidney disease but I did want Marje to check the labs. . However I've just rechecked the USG numbers and as I mentioned above the second USG could be 1013 not 1033 which is much better than 1013 and does indicate an issue.
Also the creatinine is at the top end of normal but I seem to remember Marje saying that it should be lower than what they say is "normal"
If the phosphorus levels are normal there is no need at add a phosphorus binder but you do need to look for low phosphorus foods. Weruva site has all the phosphorus levels listed and you should be able to find a low phosphorus, high protein food for Maxi. Lisa Pierson also lists phosphorus levels below link. Look for phosphorus levels 250 to 200mg phosphorus /100kcals

I added the USG levels for October and January.
I thought I had figured the food out, but I'll look at Dr. P's list again. What number is considered high protein?

Yes, Jan USG was 1033. That's good, right?

Here are the foods I got for Maxi and the levels per Dr. P's chart. Please let me know if these foods are right for Maxi. Thank you.

Maxi is drinking like crazy this evening. So worried.
 
Those foods all look good. Rotate them so there is a variety of the protein and fat. There is a good variety of carbs under 10% too. You don’t need the phosphorus binder with those.

Max may be drinking more as his BGs are higher from the furshot this morning.

Yes that is very good that the USG is 1033 and not 1013. . An improvement from Oct.
The creatinine, SMDA and the BUN are all better in January compared to Oct last year.
Was Max sick when you took him to the vet in October?

I'm sort of scared of not giving him the binder since the vet told me to. This is not to say that I believe in the vet more than you. To the contrary. That's why I'm here. I'd just like to ask: Will getting a phosphate binder when he doesn't need one hurt him?

I took Max to the vet in October because I had noticed he was drinking more.
 
Maybe those levels were better in Jan. because since Oct. he had been on special kidney disease food...?

It’s possible, but the current thinking is that feeding a cat with early stage CRD a low protein diet is old thinking (it causes muscle wasting) and the thing that needs addressing is the phosphorus levels in food. Now the recommendations for early stage CRD is to find food that is lower in phosphorus as you have done with that list.

I would wait to see what Marje says about the phosphorus binder. I don’t think you need it at the moment but I’d rather hear what Marje thinks.
