Day 2 on PZI

Good morning everyone
hope everyone had a better night than us!
Shakes has been high all night so I upped him to 1.5 units and I am praying that I
did not miss.
He does not like these needles because they are bigger than the u100

anyway, I am going to try to get some sleep

WE HAVE BLUE AT 3.5 HOURS!!!!

ITs only 196 at 3.50 but still, I am very happy to see that Blue!!
I will check again at 6 hours, unless you think I should check sooner :-D

We had a nice day, he was pretty active today
(usually sundays he is in the 300's all day on lantus)

attached are his numbers
not bad, I think for day 2
he stayed at 196 for quite a few hours but he is back in the 300's again

Should I raise to 2 units tonght, since I will be home to watch.
orkeep at 1.5 units
tomorrow is a very busy day and I will not be around at all from shot to shot

any advice would be great

You can afford to go up in dose based on your numbers, but I tend to be more cautious. ECID. Some take a few cycles to react to a new dose - it is possible that he will go lower the next cycle. Some react the first cycle and that's it. This is where not having a protocol makes things more difficult. You get to decide. You could be agressive and up by .25 (Do you have the U100 needles yet so you could do this?) or you could give it a cycle or two at 1.5. I think it would be best not to go up to 2 units. For me, that is too big of a jump when you are seeing movement.

What does your gut say?
 
ok, not really sure, I am just so happy that he is up and about i want to keep him feeling good
I will keep him at 1.5 until tomorrow night and see what number he gives me Monday night
to determine where to go.

It is really hard because I have both the u40 and the u100 needles but neither of them give me the
half or quarter numbers so I am really eyeballing it.
so the only good thing about the u40 needles is that I can eye ball it better than the u100

You probably have already seen the pictures on the Lantus site for mini doses. They do help you figure out where on the needle it might be - just remember that the Lantus dose is not the converted PZI dose. (Hard to get your head around....) the syringe pictures are about half way down the page.

Some people use magnifying glasses to figure out .25 doses. It does get easier the more you do it. :-D

With PZI they can get better #s often on cycle 2 or 3 (sometimes 4, occasionally later than that) of a dose, so typically if you are increasing doses you want to hold 3-4 cycles before raising. Some suggest longer, and I agree with that if the picture isn't clear. If it's clear though by cycle 3 or 4 that this dose isn't enough, then I would raise. The typical patterns are:

1) You get a WOW factor on cycle 1, then less good #s on cycle 2 & 3. That's your cue the dose is too low.

2) You get some or no progress on cycle 1-3, and continued improvement or sometimes a major breakthrough on cycle 2, 3, or 4.

So that's why you want to generally hold for a few cycles to see if you will get that improvement. If that is brewing and you raise the dose in the middle of it, you can get overly good results (or wonky results if their body reacts to the dose).

Especially since this is a 0.5u increase you would want to see if the improvement will continue, since that's a decent increase in the overall insulin amount - really going from 2u total in the day up to 3u.

I know you are already planning to hold the dose :) just wanted to build on what Sue said and explain the logic behind it for you & any newbies who may be following along.
