Dagron went to the specialist today

We saw an internist (internal medicine) today. Unfortunately, she didn't have an answer for why cats' numbers are usually highest AMPS. Oh well.

She seemed unconcerned about the low % of carbs in his food. The food Dagron and Gryphon currently eat is 71% protein, 1% carbs. His normal vet thought this was too low.

At first, she wanted me to do a bunch of testing to rule out infection - urinalysis, urine culture, bloodwork, abdominal ultrasound, and chest x-rays. I told her I have no money, so she said "well, you could just bump him up to 2u for two weeks and then do a curve."

It's strange though, when I talked to her about dentals - and whether she thinks it's okay to do dental cleanings on diabetic cats - she said she thinks it's fine in Dagron's case, but that she doesn't think that is affecting his BG because he isn't on a really high dose of insulin (no insulin resistance). So if she's already saying he probably doesn't have an infection because he doesn't have insulin resistance, why is she suggesting all the tests?

Anyway, I am going to bump him to 2u bid and do a curve in two weekends. Can't do it sooner than that.

The one good thing that came out of this trip is that she thinks the numbers are not as important as symptoms, which is how I tend to operate as well. Although, she also tried to tell me not to test him so much, and I'm like... yeh, hypoglycemia, no. At least I learned why vets have a tendency to tell us that - it can make the cat aggressive and difficult to test (which it is doing with Dagron, but only near mealtime).

Here is the link to your previous post, so people can follow the history. http://www.felinediabetes.com/FDMB/threads/24-hr-curve-advice.182778/

At that time I suggested you hold the dose because his nadirs were in the desired range. Many kitties have higher AMPS due to Dawn Phenomenon, same as humans.

My dentist was OK with giving Neko a dental when she was unregulated. Maybe you should try a dental specialist if Dagron really needs a dental. Bad teeth can make it hard to get regulated. But you can't say for sure he has no infection without tests.

What causes Dawn Phenomenon though? Looks like I will have to try out the Google machine.

Ever since that 24 he curve I did, his numbers have been higher. I had been thinking about bumping him back up to 2u because of that.

Here is a link to Dawn Phenomenon.

Without tests last night, it's hard to say if he went lower and is bouncing again. Since he went up mid cycle today, I would guess he started another bounce. If you were following TR, which you aren't, you would increase. But on SLGS you would hold the dose and wait for the bouncing to ease off.

I'm positive that his high mid-cycle number today is because of the fur shot this morning. I don't think numbers from today are going to accurately reflect anything at all.

Most mammals have a natural biorhythm involving when catecholamines (i.e., stress hormones) are naturally released. It's typical for these hormones to be released in the early hours of the morning in order to prepare the body for the stress of the day. As a result of the influence of the catecholamines, BG numbers rise in the AM -- hence the name, dawn phenomenon.

Sorry about the fur shot. It'll help us if you put something like FS beside the dose in the units column of the spreadsheet so we see right away something that impacts the depot. Good luck with the increase.
