Continuous glucose monitoring

Hi I have also posted this on the Prozinc thread but thought it might be better here.

Does anyone have any experience of continuous glucose monitoring as this has been suggested to us by a specialist vet alongside xrays and a full urine tests as after nearly a year we have been unable to stabilise Tinkabelle and presents as resistant to both Caninsulin and Prozinc.
I am concerned if this is just too invasive but no other suggestions are being given. We have looked carefully at her food and she still has high numbers every time we do curves and pre shots. Apologies I just can’t get my head around how to upload her numbers... usually range between 15-20 mmol (300-400mg/dl) of not higher
Any advice would be appreciated

I cannot access your spreadsheet. But has she been tested for IAA and/or acromegaly? What was her highest dose so far?

Mr Kitty was diagnosed with insulin resistance yesterday, as you can see I'm up to 14U and finally starting to get good numbers (for now).
 
Hi Jenny,

Are you referring to the Freestyle Libre that is implanted on the cat that you can scan with a phone? If so, you should definitely try it as lots of kitty caregivers here (including me!) have used it. The CGM will give the vet more information than blood pricks will over the course of many days. Some cats tolerate it very well, whereas my kitty really did not like it and removed it before the 14 day duration was up. There is a very thin filament that goes into the kitty's skin, just deeply enough to get at the interstitial fluid. The vet will apply some skin glue to keep it on and you will have to buy a shirt or two to put over it so that the kitty won't scratch it off. If you can put it on Tinkabelle's neck, the best place for it, you can get a Kitty Kollar to put over it.

Thankyou for your reply. No she hasn’t been tested for either of these. Her highest dose on Caninsulin was 8U and on Prozinc now it’s 4U in a U40 syringe.

Many thanks for your reply. I am not sure what the type is or which one they use in the UK. Our vet did say for 2 weeks so I think this it may be the freestyle Libre or something similar. She is a very anxious cat so I am sure the readings will be better. They said it might be better to sedate her to put it on and do the X-rays as she get cross very quickly being handled.

One of the caregivers who contributed to the thread I sent to you is in France and she is able to get it there so I assume it's the same in the UK. Abbott is the only company that manufactures it as far as I know.

We cannot see your spredsheet

To set spreadsheet sharing permission:
* At the top right of the spreadsheet screen click on Share.

* In the pop-up dialog box click on the Get Link section.

* Change link access from 'Restricted' to 'Anyone with the link' and make sure that the permission on the right hand side is set to 'Viewer'.

* Click Done to save the changes.

That is a very high dose of Caninsulin. I would get her tested for IAA and acromegaly if you can afford it, I believe the Royal Veterinary Clinic is the only one in UK that can do it (I'm not sure if they can do IAA though). To be blunt - she would probably be dead if that Caninsulin dosage was wrong, so I do suspect there's an underlying issue.

Why is she down to 4U on ProZinc? Did she earn reductions? Or did you drop her dose way down when you switched?

Apologies, I have checked previous records and she got up to 5U on Caninsulin and the vet felt that Tink was resistant to it so swapped to to Prozinc, she started on 3U and then we have been slowly increasing. At first her bloods were coming down a bit and then quickly started going up again and have remained quite constantly high since then. Tink has been referred to both RVC and North Downsand currently our vet is consulting with one of their specialists who has recommended further tests and using the continuous monitoring readings for two weeks to get better readings.
It’s been a difficult time x

I understand! If you look at my spreadsheet you'll see I did something similar. Started on Caninsulin (Vetsulin in the US,) then tried ProZinc, and eventually Lantus. Ultimately I don't think it mattered what insulin I used, I just had to keep increasing the dose until I broke through. Definitely ask about the IAA and acro tests!

Many thanks, I will do, she is having the monitor put on next week and they are doing some further tests at the same time. Fingers and paws crossed xx

Thankyou so much for the link, you have provided a lot of answers to my questions and I must very grateful. Tink is having it put on next week under slight sedation. The implants are more expensive here but we will give it a go as a one off and go from there...hopefully she will keep it on! Xx
 
I have ordered the Libre from Abbott uk- waiting for delivery then getting fitted at vet hopefully this coming week.

I have been advised of a kitty Kollar and will make something similar, I wondered if the plasters made for them will work or just fall of as for skin?
 
I think they are looking fur slightly overweight to obese who have been diabetic for less then 9 months - are you just outside that window?

Many thanks for the information, I had a look and she is quite a small cat despite eating sooooo much. Libre is being fitted on Monday x
