I'll probably be shot down in flames just for asking this but has anyone either intentionally or accidently suddenly stopped injecting insulin and monitored the results? Or significantly reduced the dose by more than 0.5IU without being prompted by a prior BG nadir reduction?

I do assume it would be an unnecessary and harmful shock to the cat so please don't reply just to say that, I'd rather hear about actual experiences or receive clear reasoning.

I did it with my cat, but he was on PZI, not Lantus. Not due to advice, I just had a gut feeling that Bob was on too high a dose at 4u BID. My gut was right in his case.

I've seen a few Prozinc cases where people on a higher dose as advised by their vet did a "reset" and started over at 1u. You might find some cases on the Relaxed Lantus forum where the same sort of "reset" was attempted. I seem to recall seeing a few in the last two years.

i've intentionally stopped insulin for a few cycles at my vet's request when he thought alex might be over dose. it didn't pan out the way my vet thought it would and i resumed insulin at the same dose. i've also skipped a shot here and there when i felt there was too much "depot" action remaining after taking a dose reduction, but i don't think that's what you're asking.

withholding lantus for approximately 3 days to *see* where kitty is at was a technique used by some prior to widespread acceptance of the Tight Regulation Protocol with Lantus or Levemir on the FDMB. if the TR protocol currently followed by most in the Lantus TR ISG is properly followed from the start (using the dosing guidelines suggested in the TR protocol), there's really no reason to check to see if kitty might be over dose. the TR protocol offers a very systematic and methodical approach to dosing.

this technique is not to be confused with what carl mentioned as a "reset". imho, a "reset" is when there's already a strong suspicion of kitty being over dose... usually due to any combination or all of these things: the initial starting dose was set too high, subsequent dose adjustments were made early on in increments greater than 0.25u or 0.5u (lantus/levemir) and/or there have been missed reductions... usually more than one. under those circumstances, there have been some lantus/lev users who have *reset* at a much lower dose.

I once reduced my cat's PZI dose from 4 units to one unit because of a feeling that his insulin dose might be way too high. (But in our particular case all that showed was that he wasn't on too high a dose... :-| )

I was fortunate too that my cat didn't have any history of ketones. When significantly reducing an insulin dose in a cat who has had ketones I think it would be prudent to monitor the pee for any sign of that. (I don't know whether your cat has or hasn't had ketones).

I don't know the 'cold turkey' song: Could you hum a few bars...? ;-)
