Hi all! Hansie cat is doing well, his levels have stabilized and he has picked up weight. I managed to locate somewhere I can purchase lantus. He is currently on humulin, 4 units twice daily. I would like to change to lantus, how do i determine the right dosage? Humulin works for over twelve hours, whilst lantus works for 24. Any advice?

Um, you've read the info for humans.
In cats, Humulin works for 6 to 8 hours and Lantus works for about 12 hours.

To estimate a starting Lantus dose:
What is your cat's ideal weight? What is your cat's current weight?
Take the lower of these.
Convert to kilograms (pounds/2.2).
Multiply by 0.25.
Round down to the nearest 0.25 (we eyeball 0.25 and 0.75 on syringes).

Lantus doses are increased based on the nadir, the lowest glucose between shoots, NOT on the pre-shot numbers.
New users should test before giving insulin and read 200 mg/dL before shooting.
If the cat is below that, stall 30 min without feeding and re-test to see if the glucose is rising.
