Caninsulin dose advice needed in an hour

Absolutely DO NOT give him the same dose, Sue. The dose is too high and that 43 was w-a-y too low. (FDMB uses 3.8/68 as the hypo threshold for Alphatrak meters.) The 12.8 could be a bounce off the hypo.

I'm just going to look at Romulus' spreadsheet. Back shortly ...

EDITED TO AMEND:

The 12.8 is influenced by the high carb food and the honey given at AM+9.

Question: was the PM dose for 16 August 4.5 IU as recorded in Romulus' spreadsheet, or is this a typo? (It looks like a typo but please confirm.)
 
Sorry, it was a typo. I only put his SS up yesterday and got confused with Frankie's dose!! I'll change it.
 
Sorry for taking so long to get back. Got held up unexpectedly. What time is PMPS? I think it is still likely to be influenced by the high carb food and it's possible you may get higher numbers from a bounce on tonight's cycle.

Are you able to monitor through the PM cycle, Sue?

In case I miss you again, if Romulus were mine, I would not give insulin at all if it is a low PMPS. This is suggestion is based on the assumptions that you are checking for ketones and that Romulus doesn't have a history of throwing ketones; also that he is eating OK and doesn't have any current infection or inflammation issues.

The maximum I would give if Romulus were mine would be 0.5IU Caninsulin and then I would want to monitor until safely past nadir. It may be too low a dose but if Romulus' pancreas is sputtering it's better to err on the side of caution. Other members may have different suggestions.

More questions:

1. Did Romulus eat less during today's AM cycle?

2. How long after he last ate did you get the 2.4 mmol/L reading?

PMPS was an hour ago. Just tested and he is 15.1. The 2.4 was three hours after eating, but he only had a small amount then, nothing like he usually eats. I just fed him again and his appetite is back to normal. I will shoot once he is finished.
 
There's another thing that is worth noting. The high carb and the honey didn't raise his numbers much in the first hour after he ate them (assuming that there was 1 hour between the 2.8 and the 4.4).
 
I gave 0.5. I can test for as long as necessary - I can sleep at work lol!!! He has no history at all of ketones, and I think it was slightly less than an hour between HC food and retest. It was the sitter who came back to check on him after getting 5.4 at 1pm so not entirely sure of timings.

One thing I remembered but I can't even remember if this happened this morning or yesterday, but as I injected Rom I got a feeling that I had already injected him. If it was this morning, that accounts for the low reading. He will probably be high AM tomorrow, so I will inject 1u then as see where that takes us. I just wish I knew if this really happened or not!! I'm such a bad mum.

I think the dose reduction in the morning is a good idea, Sue. Remember that today's hypo may leave Romulus more sensitive to insulin. If he was mine, I'd be very conservative with dosing.

Thank goodness indeed. Yes she fed at 5.4 but that was when he didn't eat much. I asked her to pop back after her next client which she did happily. I think she is just as invested in the cats as I am!!! She texts me the readings and I told her what to do.
 
You're blessed to have her. :) I'm housebound because of my PTSD, etc. I don't know what I'd have done if I'd had to leave the house when Saoirse was on insulin. Even being home all day I found it a real white-knuckle ride at times (but the PTSD influenced that).
 
Well Romulus went out and didn't return until this morning, so I couldn't test!! He was 13.2 this morning (238) so I gave him 1 unit. I will stick with 1 unit unless his numbers start increasing.
