Buprenorphine in hyperthyroid/hypo civie

Kitty was diagnosed with resorptive lesions and had 4 extractions today. He was given buprenorphine to take once every 12 hours. My question is this: It says in my research to beware if the cat has an UNDERactive thyroid. My cat is HYPERthyroid; however, he is now HYPOthyroid as a result of the medication he is taking, so he technically has an underactive thyroid as contemplated as a problem on the package. I understood he would feel drowsy from the drug, but he seems rather active (almost antsy), and this is the day of the anesthesia. Thoughts?

that suggests that it can cause hypothyroidism, or make the thyroid more hypo.

Personally, IMHO, it is the best for pain relief, and after all those extractions, will likely need it. You probably wont have to give it for too many days. Just keep an eye out for more hypOT symptoms. Like any drug, they have to warn people of anything that has come up. A Lot of Merlyns heart meds said "use caution in diabetics" etc.

Hi

If your cat is on methimazole for hyperthryoidism, he should be euthyroid (in the normal range), not hypo. I'm not sure if that is what you meant. Anyway, hypothyroidism is associated with a slow metabolism and that may be why there is a warning. Buprenex is a drug that has a pretty wide range of safe use so as long as you look out for excessive sleepiness or dysphoria (disorientation, seeming "high") I wouldn't hesitate to give it to him. Extractions are painful for cats just like for us, they just hide it much, much better.

Hi Jess and Earl! Thank you for responding! He is hypo. They have brought his thyroid past the normal range and into the hypo category and assured me this is okay in cats. He is so skinny and always has been that I hesitate in cutting his methamzaole down further becuase I am afraid he will become skinnier. Thanks for the response!

I have no idea why your vet thinks its OK to be hypo. My Conor had I-131 and was sl hypo and I waited a while to get him retested but he was still hypo and my vet has him on Thyrosyn generic synthroid. My vet said being hypO is hard on the kidneys like hyper is. If hes skinny, maybe theres something else underlying that besides the thyroid. Does he eat enough?
 
What was his last T4 that you say is in the hypo range? Idexx's T4 results includes the following information:
Interpretive ranges:
<0.5 subnormal
0.5-4.7 normal
2.3-4.7 grey zone in old or symptomatic cats
>4.7 consistent with hyperthyroidism

Cats with subnormal T4 values are almost exclusively euthyroid sick or
overtreated for their hyperthyroidism. Older cats with consistent
clinical signs and T4 values in the grey zone may have early
hyperthyroidism or a concurrent non-thyroidal illness. Hyperthyroidism
may be confirmed in these cats by adding on a free T4 or by performing
a T3 suppression test. Following treatment with methimazole, T4 values
will generally fall within the lower end of the reference range
(0.5 - 2.3)
