So..since I can see remission is not only possible with Boomer but likely and could happen reasonably soon I think......I have a question.
When do we declare remission? Is it when he stays green consistently without any insulin even though his BG may vary within the green? Is it when he drops to a certain BG (40?) and stays there without insulin? Is it when his BG remains steady even though he's eaten, indicating that the pancreas is producing enough insulin to counteract the carbs he gets in food?

We consider a cat to be in remission when they've gone through an OTJ (off the juice) trial which is 14 days without insulin and still in normal numbers (below 120 and majority of time in green).

The Lantus depot can still affect up to 6 cycles after the last shot, so that could still be helping Boomer's numbers. However, he is looking very good. :mrgreen:

Sienne was gone last week when Boomer decided to scoff at my advice and take you on an extended trip in low numbers.

While we quite often do go to the 0.1u dose Julie mentioned before they completely go off insulin, there have been some cats that just were done at 0.25u and occasionally even 0.5u. What we are going to want to see is how long the depot from the 0.25u dose keeps him in green. Normally, for a cat ready to go without insulin, we'd see a drop in numbers three hours after being fed. But it's not a one size fits all as Boomer has proven.

I think I'd let him go at this until you are at least six cycles past the 0.25u dose OR if he starts trending up into blue. If starts trending up, I'd start shooting 0.1u. If he stays in green three more cycles after this one, it will be six cycles past the 0.25u dose. We might want to think about starting a trial then but let's see how he does.

Thanks Marje! Everytime I test Boomer now I hold my breath lol. This morning I thought for sure he would be in the blue! You can imagine how happy I was when he was 79! And yes I realize this streak could end because it could be the depot that is keeping him green although I've never noticed the depot lasting this long with him. Time will tell! Even if it doesn't happen this time, I'm confident it will happen.

So....what is different during the OTJ trial?

.1u of insulin is really hard to measure!

Is Boomer dropping too fast? He's due to eat in about 45 minutes. I think I'll test again at 3.5 and perhaps feed him if he's dropped much further. Any suggestions????

I don't think he's dropping too fast. But I would definitely feed him.

Usually when they first drop down into green, it's a good idea to give them a couple tsp of LC food. So if you haven't fed yet (it's not +3.5), I'd feed now. And then I'd grab another test about 30 mins after you fed.

I'd probably give him something a little higher in carbs and retest in 30.

Well he had already eaten most of his meal but I gave him a tblsp HC FF and persuaded him to eat it.

Unless he hasn't had all his food, I'd see if he keeps coming up on his own. If he hasn't had it all, I'd switch to LC if you haven't already.
 
He has a wee bit of food left, a mixture of the two so MC. Maybe a teaspoon. I think I'll leave it out for him. His next meal is +3 from now.
