Bob - AMPS 392/ +5 320/ +8-MDPS 295 (gave 1.5u)

Not a good day for us. Bob scared me this morning. Found him laying in the kitchen next to his water bowl showing little signs of life.
At 5am he got 100 ml of fluids, but would not eat. And I had no choice but to go out the door to work at 5:30.
All day long I worried about DKA or fatty liver. Checked on him three times during my shift, but no big improvement.

Went to town, bought some a/d to syringe feed him. Vet also gave me a shot of cerenia for him which he got as soon as I got home. Then I paced until the UPS guy came at three with his insulin.

Bought some ketone sticks and pediatric suppositories to use for his constipation.

At 4:00 I tested him - 413. And gave him 2u. He'll get fed at 6, tested then and at +4. If I don't see a drop, I won't wait twelve hours for his next shot. I'm scared. Trace ketones on the pee stick. He'll also get more fluids tonight and syringe fed until I can get at least a half can into him.

Gonna be a long night, but I am off work until Sunday noon.

Gosh, Carl, hope the insulin starts to work right away and that he improves overall. I agree that if he heads back up after 8 or so, shoot again. Since you can monitor and know what you are dealing with, it seems like a safe plan. Yes, staying on top of the ketones is vital. As you know, he has all the ingredients for DKA.

Do you think a feeding tube would be a good idea?

Not much news, Nancy.

Still syringe feeding, 10cc at a time every couple of hours.
I did get some cypro this afternoon, so we'll see if that helps make him feel like eating.
I'm upping his dose next shot to 1.5u unless I see an unexpected low number. Shooting IM like I did last time around.
He is getting fluids twice a day.
I'm not testing like a dervish because the meter isn't telling me to. I figure a +4 or so will tell me what to do next so if I don't see a big drop, I'm giving him a break. He's getting poked and prodded and forced to do things he's not really wanting to do, so I'm trying to pick my battles and get away from it in between "steps".

I'll keep his SS up to date so even if I'm not posting much, you'll be able to see what's going on. I was going to modify it to be better suited to TID, but I don't plan on doing TID for long, so I'll just use what I have.

Got a 295 at +8, and shot 1.5u.

It could be a falling number, but if so, not by much. I'm guessing he went lower in between and is now coming up. Either way, he got his shot and I'll test again in 2-3 hours when it should be onsetting.

FYI to anyone who isn't familiar with Bob's special needs....
The last time he was on insulin, he was also DKA. He was getting 200ml of sub-q fluids a day. My vet advised that instead of shooting sub-q, due to a couple of reasons
1- given his state of dehydration, she didn't believe that sub-q shots would absorb like "normal".
2 -if the fluids were given too close either location-wise or time-wise to his shots, she thought the fluids would screw up absorption of a sub-q shot...

she instead advised that I give him his shot into muscle (IM), and even after the fluid therapy was done, I continued to give IM shots the entire time he was on PZI. Because that was what I was familiar with, and because it was working. His curves weren't the same as other cat's curves. The insulin didn't last 12 hours most days. It absorbed quicker, and reached onset quicker. He usually nadired around +5 or so. Got maybe 10 hours out of a dose.

Same routine this time. He's getting sub-q fluids twice a day. So I am shooting into his leg muscle just like I did last time. It worked for Bob. No one should ever try this without the approval of your vet!!! I've never advised anyone else to do it the way I do it.

The only thing I am doing differently right now is shooting TID. I won't do this forever, but I want to try to keep his numbers as far down as I can since he isn't eating well. I am concerned about DKA and about Lipidosis. So I am being aggressive.

I have never been happier to see a yellow number in my life. ;-)
 
Yes, we all LOVE yellows! Bob will come around, he did once and will again. Yes TID helps level it all
out .... we have been doing for 3 yrs. now :) of course lately her pancreas has been sputtering and I
am telling her, STOP IT! A big gulper like Payne doesn't need a pancreas sputtering now but it is
interesting to watch, harder than hell to dose! Keep the faith Carl ....

Carl, just popped in to this forum to see how Bob is doing and am so pleased to see that his sugars are coming down.
TID sounds like a good plan. (Did that with Bertie for about a year and a half, though have NO idea how Nancy has managed it for 3 years...)

Hoping and praying for swift and steady improvement for Bob (C'mon, Bob - get well for your Dad!)

Take good care of yourself too, Carl: Good rest, good food, and plenty of water to flush all that adrenalin through your system.

The 1st dose of cypro didn't do it, but hoping today's does. He's less resistant to the syringe though.

I just bought the ingredients for the "liver shake"and about to make it...

So far today he's more responsive and less miserable looking. Praying it's the first step in turning things around.
 
I even minced some. He sniffed, but wouldn't take a bite.
 
Not that desperate yet.... I can keep on syringe feeding for a while before work makes it impossible.
 
Just want you to know I am thinking of you and Bob and hoping he is feeling a little better. Hugs to you both!
