Ok,

My Civvie Slappy decided one Thursday evening to not eat, throwing up and hiding under the bed.....We have been to the vet, had extensive blood work done and the vet told me not to worry about much, that my 15 yr old looked good...a few things off, but she was actually good.....Since then, we have started eating, stopped throwing up and no more diarrhea (which came later in the game). She decided during her 'I am going to eat again' to like sardines (think that is why she got diarrhea). We got a script of Popectalin and gave it to her once and have not needed to give it to her again... She has been back to see the vet once before yesterday...he gave her b12, fluids. She seemed to be doing fine.

I had her on a floating appointment all last week, canceling and rescheduling daily and decided to keep the appointment I made on Wednesday for this Saturday regardless.....She stopped eating again by Thursday! UGHHHH!

Yesterday, the original vet was not there and we saw another vet (I did not know this when I scheduled). She printed off the blood test results and accidently left them in the room, which I grabbed and have with me. During the visit, she was given more fluids, b12, 7 day antibo., nail clipped and sent on our way. She also informed me that by looking at the blood work, Slappy could be having problems with her thyroid.....The HE vet never mentioned it! She gave her a pill, and I am to get cream to put in her ears daily...She said that a thyroid problem usually makes you eat more but not uncommon to not want to eat either!

That is why I am posting here to reveal some of the numbers to see if anyone could offer some help on the high/low counts:

T4: 6.5 - normal being .8-4.7...high
WBC: 3.1 - normal 4.2-15.6.....low
HCT: 46.5 - normal 29-45.......high
LYMPHOCYTES: 11 - normal 20-55........low
MONOCYTES: 15 - normal 1-4.............high
EOSINOPHIL: 1 - normal 2-12.............low
AST: 82 - normal 5-55 ....................high
CK1: 4671!! - normal 64-440.............HIGH
BUN: 47: normal 15-34 (he said probably from throwing up for 2 days/dehydration) ...................high
GLUCOSE: 163 - normal 70-150 (he said probably from throwing up for 2 days/dehydration)...........high
B/C RATIO: 47 - .............................high
ABSOLUTE NEUTROPHIL SEG: 2263 - normal 2500-12500 .....................low
ABSOLUTE LYMPHOCYTE: 341 - normal 1500-7000.............................low

I do have all the rest of the numbers from the test...these are just the ones that were either high or low......

I NEED HELP UNDERSTANDING ALL OF THIS!! DOES THIS MEAN SOME KIND OF CANCER???? During my research, this is what caught my eye the most:


Monocytes....low level
•Monocytes develop and are stored in the spleen and bone marrow. They secrete proteins used to clean inflamed and irritated tissue, as well as eating foreign material. Monocyte levels usually do not change unless there is a cancerous leukemia present in the cells.

Lymphocytes...low level
•There are two types of Lymphocytes; B cells, which produce antibodies and attach to and destroy invading organisms or other foreign materials and particles; and T cells, which memorize foreign objects and identify them in some way to the body. Lymphocytes are produced from lymphoid tissues such as the spleen and the lymph nodes.
If the level of lymphocytes in the blood is low, this is called leucopenia. Leucopenia can be caused by chronic diseases such as kidney or liver failure, chronic pancreatitis and hyperadrenocorticism (Cushing's disease), as well as medications including chemotherapy drugs, prednisone and other glucocorticoids.


Eosinophils...low level
•Eosinophils are produced in the bone marrow. These cells help to fight off parasites and eat up bacteria particles in the body. Eosinophil numbers will be low if there are conditions present that cause extreme or prolonged stress.

Creatine Kinase (CK) and Creatine Phosphokinase (CPK)......VERY HIGH LEVEL
These are two slightly different names for the same muscle enzyme, which is released when muscle is damaged. The US laboratory, Antech, mentions that this value can also increase in anorexic cats.
In most CRF cats, there is usually only a mild increase if any, and this might simply be because of the stress of being held and having blood taken at the vet’s; occasionally, levels also increase after prolonged inactivity or if a cat has a seizure. Many labs have a maximum level of 300, and a level of up to 500 is not usually cause for concern.
If levels are high, it may indicate some kind of muscle disease; alternatively, very high levels of CK are often seen when an animal has heart issues. If your cat's level is over 1800, your vet should investigate further.

I am soo sorry for this being so long, but I am soo worried for her, and a might pissed with the vet for not even bringing any of this up..... I need to know more about what is going on! So, of course, I turn to the furry family that understands more of my anxiety than anyone else!
 
Gustav have Multiple Myeloma, blood cell cancer. And his chart has been like this

always have had leukopenia - too low white blood cells

started getting very high plasma proteins + plus 3+ in urine protein

started to get too low albumin protein


This is typical with leukemia, and the blood cell cancer BUT also with the degenerative SLE systemic lupus erythematosus, so they can easily be mistaken for one another.

In addition we also did heart ultrasound scan and internal organ ultrasound scan, looking for abnormalties, as perhaps a growth in the liver, pancrea, kidneys etc but all that looked fine. And the only way to find out of Gustav really has blood cell cancer, is to put him under and drill a hole in one of his legs for bone marrow sample. I chosed not to do that with him.


But Slappy seemed to have hyperthyroid T4?

The first I heard about high level for thyroid was yesterday by the other vet!

They also did a Urinalysis and there was a trace of glucose, blood was +2, RBC 6-10 and normal says 0-5... No one really went over any of this with me, he vet just said it looks really good!
I know I would have remembered if he would have! Slappy does have a small 'thing' on her chin, it scabs up, scab falls off, it scabs up...etc....it has been there for a while, but never got bigger. Mommy was buzy moving from state to state (2 times in 6 mo - over 8 hours each way), and has not had it seen about.......feel sooo guilty! He vet says he will remove it, sew up the small spot and have it looked at for cancer.. She has developed a 'twitch' with her eyes and head... it is now kinda affecting into the front leg area... dont know if any of that has to do with anything????????????

I feel like I did with Gumpy...not knowing anything and guessing at most of it!

Then get back to Texas, Bean very sick...diabetic, then 2 wks or so later, Gumpy starting to have seizures.....at the bridge now :cry:

Slappy was next in line, she just beat me to it....

Gumpy cost soo much and I am maxed out! What the hell! Sorry but mad at myself, upset and worried over my Slappy girl....I dont know if when I treat the thyroid with meds, will the other numbers adjust back right????

I dont know what all of this blood work up is!
 
To me it looks like she has an infection somewhere. I cannot tell if it is infection vs. cancer though. I would also run a full thyroid panel since it looks like she is hyper thyroid as well. The CPK values and slightly elevated BUN/Creat could be due to not eating/drinking much. Malnutrition/anorexia can cause values like this since the body goes after the fat and muscle stores for energy, which also increases the work load on the kidneys.

What sort of antibiotic did she give? I'd ask for a good broad spectrum one. Zeniquin comes to mind. Get her eating - what ever it takes. Did they run a full thyroid panel before you left or are they planning on treating her hyperT?

The antibiotic was Covenia Injectable. That is all I know about it???? What is a full thyroid panel? I know that they are giving us the cream and I get to pick it up tomorrow that gets rubbed in the ear for her thyroid and she mentioned something about radiation to get rid of the tumor???? So, she knows this without an ultrasound or xray?? Can that be possible?
She has eaten some today, not a lot, but alot compared to yesterday which was 0, and the 2 days before......I tried to syringe feed, and CANNOT DO IT....she was in 3 towels and would not let me .... I found an eye dropper that I have filled with nutri cal, waiting to do, but she is trying to eat, so I do not want to stress her out.... we will see what happens tomorrow a.m..
I also bought some chicken baby food that I tried in the dropper and it will go thru, so I will do that as well...I cant get the syringe to work that the vet gave me, it is just too big and I cant rassle her, and try to push the plunger that wont push anyway...I also tried a turkey baster, just tipping it up for seconds and that did not work either...I DESPERATELY NEED A SECOND SET OF HANDS....
I am hoping if the thyroid gets under control, she will calm down and not be soooo nasty... She only weights 6# and that is 6# OF TOTAL FIRE right now! WOW! :twisted:

Damn, why do vets always go for convenia ohmygod_smile ... It is not a good broad spectrum antibiotic. It is labeled for skin infections. It is a two week shot. Keep an eye out for side effects within the first 2 days. The most common side effect is extreme lethargy. Dr. Lisa has a page on the antibiotic on www.catinfo.org. It does not cause issues in all cats, but it can be nasty in the ones it does and it isn't something you can take back. You have to deal with the side effects until the drug starts to wear off days later. Next time you go back, have them write in red in your chart No Convenia. There are better drugs out there.

Now onto the thyroid - if your vet is confident in the hyperT diagnosis, then no additional blood work is needed. Most of the time they will run a free T4 equilibrium test and TSH test to confirm things. The radiation treatment she is talking about is radioactive iodine and it kills off the non-malignant tumor on the thyroid causing the issue.
