Blood in urine but no UTI???

Greetings everyone - I know it's been a while. I used to be a regular over in Lantus Land before Leo's acromegaly dx in March 2011. After some trial and error, I've been able to keep him mostly in the blues on 8 to 16u of Levemir bid and life has been good.

Seven days ago I started getting high flat pink BG nonstop. Knew something was up, and then this morning I found a puddle of pink urine next to the litterbox. My civies are both healthy, so I called the vet and they said to catch a sample from Leo if I could, in case they couldn't get one at this appointment this afternoon. I did, and yuk! It's the color of Hawaiian Punch!

Vet did physical exam, urinalysis and complete bloodwork, and found...basically nothing. Tons of red blood cells in the urine, or course, but no white blood cells or bacteria to speak of. Bloodwork all within range - one number was slightly off - will get the paperwork tomorrow - but electrolytes, kidney, liver, and pancreas values all OK. Physical exam found no masses in the bladder (which was empty because he had just peed at home), kidneys, or lower abdomen. Temperature a bit low, 99.4, breathing and heartbeat normal.

I've watched him pee and there's no straining or pain. His appetite is great. He's drinking less water and peeing slightly less than he normally would with BG in the 300s, but vet says he's not dehydrated. We decided to treat this as an atypical urinary tract infection and started 6.25mg Clavamox (antibiotic) this evening. Vet is stumped. Folks at Feline Acro group are stumped. I'm stumped. Does anybody here have experience with this type of situation? I'm open to any and all ideas!
 
Not that uncommon. My Patches has it sometimes. It is called sterile cystitis. Patches seemed to be due to stress like my Smokey attacking her.
 
my first thought was cystitis too as one of my boys had blood in his urine once a few years ago. he also was peeing out of place though (on my fiance) and after a full workup there was no infection or anything out of the norm either. his official diagnosis was idiopathic cystitis, most likely due to stress. (i remember he was sent home on a couple meds, one was temporary and the other we ended up having to change because it made him a zombie. we put him on prozac for his stress and he never had another problem.)

the blood in his urine though was slight. i would definitely say his didn't look like punch. yours sounds like much more blood so that throws me for a bit of a loop
 
Thanks you guys! I think UTI is the most likely cause. Should I assume he's in pain even if he's not acting like it? I've heard that UTIs can hurt, but acrocats tend to be even more stoic than cats in general since they have so many issues. I can give buprenex if it's warranted.
 
Did they do a urine culture? That is the best way to differentiate UTI from cystitis. My Minou has had cystitis twice and a UTI once. Cystitis will generally clear up on its own in about a week. UTIs need to be treated. Both can be painful
 
Tess has had it once or twice and it was from crystals. yes it is painful and pain meds would be great if you can give them. The other thing the vet recommended was Cosequin. Apparently it is ot only good for joints it soothe bladder irritations too.
