Bladder infection help

My 16.5 year old cat keeps having issues with bladder infection. He doesn’t show any symptoms, he’s peeing fine, no accidents, no straining, no obvious symptoms at all, but the vet said in an ultrasound there’s a lot of infection. Urintest was normal. She wants me to give him anti inflammatory pills, but I’m really bad at giving him pills. He usually ends up very stressed and just hides or is aggressive, without pills he’s the sweetest, most laidback love bug. His glucose had been unusually high, but after I started him on a berry supplement for urinary health it came back to normal. He’s been in low 100s the last few weeks. I’ve read some reviews claiming Duralactin has helped with cat inflammation, so I’m considering trying that. Does anyone here have any similar experience with diabetic cats and bladder infections? I’m grateful for any advice.
 
Bladder infections are quite common in diabetic cats.
Why did the vet do the bladder ultrasound if there were no symptoms?
It is possible for cats to also have FLUTD which is an inflammation of the bladder not an infection. Google FLUTD for more details and treatments.
Can you tell us a bit about your kitty please?
What insulin and dose is he on?
Are you home testing the blood glucose?
I seem to remember @Wendy&Neko using Duralactin. She may not be around until tomorrow now.
 
I used Duralactin for Neko's arthritis inflammation, and it did help for that. You can't use Duralactin if their calcium numbers are high or they have a milk allergy. D-Mannose is a common supplement given for urinary infections.

Which anti-inflammatory pills does the vet want you to give. At the time Neko was diagnosed with diabetes, I had a second cat diagnosed with FLUTD, specifically idiopathic cystitis. Diet is an important part of helping that condition.

I think this is the likely cause and not an infection. I do not know how one can tell an infection via an ultrasound. I can think one can see inflammation.

Here’s some more information about my boy Sammy:
Last year, at his regular check up, the kidney values were a little high, so the vet checked him and said he had a UTI. He got antibiotics, then more antibiotics, then stronger antibiotics, which were impossible for me to give him. He would gag and drool for hours, even with a pill gun. Until he would just hide under the bed and never come out. So we stopped those pills and he got an antibiotic injection, still UTI. Another antibiotic injection. Then a break. This year at check up, again his kidney values were off. His urine was normal, but the bladder ultrasound showed a lot of inflammation. (I apologize, I used the word infection instead of inflammation in my original post; English is my 3.language, sometimes I misuse words, my first language is German...) Anyhow, he was given Metacam, but got bad diarrhea from it, so I stopped it. His insulin is Novolin and he’s on 1.0 units in the morning and 1.5 units in the evening. He was getting science hills prescription diet until recently, when I looked closer at the ingredients and it seemed to be I’d been feeding him garbage (I could be wrong ‍♀️), now he gets Tiki Cat (the kind with highest protein as was recommended by a vet tech working at the pet store I buy his food). He’s a total kibble junkie, but I convinced him to eat some canned food for added hydration, he also has a water fountain to invite him to drink more; he seems to really like it. He’s had a urethra blockage a few years ago, maybe in 2017 or 2018, and was hospitalized for that. The day I got him back, I had to rush him to the emergency vet in the middle of the night with severe hypoglycemia. His glucose was so low, they couldn’t even get a reading! He recovered well and I learned to better recognize the signs of it. Two more midnight incidents of hypoglycemia in 2018 & 2019 and he’s been better since (knock on wood ). I got the water fountain after the urethra blockage and also bought a water filter pitcher for the pets after that. I check his glucose myself once a week.
I’m so glad I found this forum, sometimes it feels very overwhelming to care for him, but I’d do anything for my babies. I also own another cat, Rory, she’s in stage 2 kidney disease and I have an Australian Shepard mix, Skye; she’s Sammy’s best friend. I’m not sure if he’s in pain because of all the inflammation in his bladder. I know cats often don’t show it. He does sleep a lot and usually lays on his side. He does sometimes seem to have trouble walking and can’t jump very high anymore, so maybe arthritis too? It definitely got worse when he was taking Metacam, he’d even wobble and tip over, or walk a few steps and then his legs seemed to give out and he’d plop on his butt. That went away when I stopped the Metacam. I can’t remember the name of the medicine I was told to give him now. They are capsules and they’re also prescribed to humans for anxiety and the vet tech I was talking to mentioned Sammy would be too high after getting these to stay mad at me for long, after I said I had concerns as I am aware that I’m very bad at giving him pills... ok, sorry, long story, so I’ll stop now. I see Sammy has had a lot of hypo incidents .
Novolin is not the best insulin, it hits hard and fast
What state do you live in?
There are better insulin for cats such as Lantus or Prozinc

Metacam is contraindicated for kitties with kidney issues. Glad you've stopped that, would like to know what you are giving now. Novolin is not a good insulin for cats, it doesn't last long enough, great for dogs though but dogs aren't cats. Better insulins for cats are Lantus, Levemir, or Prozinc, all much longer lasting. If an insulin lasts longer, it keeps numbers lower for longer. On Novolin, your kitty is probably spending a lot of time in higher numbers and it's that which might be contributing to frequent urinary infections.

If your kitty has issues walking, it could be from diabetic neuropathy. More info here: Feline Diabetic Neuropathy (weak back legs). Zobaline/methyl B12 can help, as well as getting him better regulated, which might need an insulin switch.
