BG Numbers all over the place

My cat Bear is 15 years old and been a diabetic for 2 years. He had a bad hypo incident in April of this year and now I can't seem to regulate him. If his numbers are in the 400 range and I give him insulin, his BG drops to below 50, which is very scary, then shoots up into the 400s again. (Somogyi) His last dose was 3/4 unit of Vetsulin and he weighs @13 lbs. He is skin and bones and his appetite varies from day to day. I feed him the recommended Fancy Feast varieties and he also takes Vit B injections on a monthly basis. He was regulated until April and he was off insulin (remission) for May and June. While he was getting an update on some injections, we ran his urine and he was high again for glucose. I waited 5 days, tested him at home and he was in the 400 range. So the vet and I decided to go back to insulin again. His NADIR points are all over the place also. 4hours, 6 hours, 9 hours?? I am really confused.
And my vet is also.
Right now I am testing him at home after no insulin for 4 days. He has gone 324, after 4 hours -138, after 6 hours-415. I am betting he dropped to 50 or below between the 4-6 hour points.
Does anyone have any idea what might be going on?
 
you've come to the right place. folks here are great at helping with dosing and such. One thing you'll be asked to do is set up a signature to add info to (see mine below?) just click on your screename in the upper right and choose "signature".

the next step will be setting up a spreadsheet... there will be folks more experienced to help you with that along soon. ;)

I have added to my signature but am really struggling with the spreadsheet. How do I get to it and put in my data? My last curve was with out insulin and sad to say he dropped into the 60's without it. I think something bad is going on.

My signature is completed with my spreadsheet, but I am interested in what else can cause the BG levels to go bonkers in the experience of the group. My last line of data is testing on my cat without any insulin injection and his BG dropped to a 75 by itself. What types of conditions can cause this type of inconsistency?

For sure, your numbers are weird.
At that point, I would check the accuracy of my meter.
What really upsets me is the red number between a blue and a green one, without having injected any insulin.
Do you have another meter in hand? Even a human one?
 
no, I threw my Relion away because it was giving me false readings back in April when this all started. that is when I bought the I Pet because I also changed vets. My first vet just basically threw up his hands and said he didn't know what to do, (this was after the 2 Hypo incidents). So my 2nd vet used the I Pet system and I switched over to that

I work for the Chemical Industry, and when we have such strange measurements, in 100% of cases, the instrumentation apparatus doesn't work well, and has to be replaced.
I really don't know what to tell you. :bighug:
 
So, the only reason the numbers could look like this is a failure of the instrumentation? There is no other clinical reason he might be bouncing around? Ok, I will go back and have the vet retest with both our meters. I'm guessing my cat has something else wrong with him and was hoping for some suggestions from the group. I am scared he might have some insulin producing tumors and I thought there might be some people on this site with experience in that area. Maybe I need to repost with a different subject line to draw those people in?

Pretty much, yes.....there's really no other answer for going from 164 to 425 and down to 75 in 4 hours time mid-cycle like that...especially with no insulin

It's also possible you have a batch of bad strips

Unfortunately, I'm not a Vet.
So, when looking at such strange numbers, failure of instrumentation is my first guess.
Maybe there are some clinical reasons to explain such deviations in numbers, but I'm not aware of them.

Maybe you can first try to find another meter, even a human one, not to confirm the numbers themselves but the strange pattern.
 
Thanks, those are good suggestions and I will pickup a Relion today just to verify. Tomorrow I will do a curve with both the Relion and the I Pet and see what happens.
