Electra is on Senvelgo, and the manufacturer says to determine remission you stop giving the med and continue monitoring blood glucose and ketones. If it stays normal, you are good to stop. If the numbers rise, and no ketones, you can start the med again with no trouble.
This is my issue- after the last appointment we (Vet and I) thought that Electra should keep getting the medication until after her dental appointment on Aug. 5th. That is about 2 weeks away. Her +3-5 after dosing has been getting lower and lower recently though, even on a reduced dose (typically 4lb to her 10lb weight). Her pre-dose tests are not bad, but higher than a normal number. Usually those tests are at a time when she has not eaten for a couple hours, but the food is always available and if it is one of my later days (I get done with work 3, 330 or 430 each week) there is a chance she ate before I put out the fresh food. Our schedule is typically- I put out the fresh food after work and test her at the sink so she can drink, then I carry her to food, she eats, and then gets a dose soon after.
The advice I am looking for is about numbers and trends. I asked the Vet about the possibility of bouncing numbers, but she didn't think that was happening. I am wondering if I should skip doses on the days where she is so close to 120 or below. If it is a normal bg, or very close, would you skip? Barring food removal (with three cats and no help at home, and automatic feeders that cannot remove food from the dish) is there an additional time you would want to test in addition to the pre-dose and +3/4? Does it mean anything that her +4 test is close to the same numbers when the pre-dose test can be 30pts different? Maybe I need to ask other questions? Does anyone know how the dental will (possibly) affect blood glucose if she is going into remission? Dental is needed- broken tooth and inflammation issues, so we can't postpone.
I worry about the potential of a hypo as she gets lower numbers in the evenings. Senvelgo should start working within 30 minutes of the dose, and has a half life of 3.7 hrs per the manufacturer. I think that means that after 3.7-4 hr post dose the numbers should go up, not down as the medication reduces in her blood. Her pre-dose tests are usually +22/23 hrs after the last dose. I have not seen any signs of a hypo yet, but with our current heatwave she has eaten a little less and often is sleeping under beds where I can't watch her for symptoms. Today I got a test an hour before our usual dose time, because she was about to start eating after sleeping for a couple hours, and that was a 105. Testing her again in the next hour will have that food affecting it, right? So I think I should skip the dose today. I tested her urine for ketones after she ate and it was negative. Both times she was low and we skipped in the past she was a lot higher (but under 200) the next day, so I worry that she will be high tomorrow, but I will be away for a few hours tonight so won't be able to monitor if I give a dose and she goes too low.
