Lantus is very different than Novolin. You're going to need to think very differently about insulin. While Novolin is potent, it is harsh. Lantus, on the other hand is less potent but far gentler and has considerably longer duration.
It looks like you are aware that Lantus dosing is based on the nadir and not on your pre-shot numbers. The initial dose is held for 5 - 7
consecutive days. Once past the initial week, doses are evaluated after 3 consecutive days unless a reduction is indicated based on a drop below 50. If you skip a shot, give a fur shot, etc., the count starts over since the dosing is no longer consecutive. If you've not already done so, you may want to look at the

I have a couple of thoughts regarding Gilbert's spreadsheet (SS). First, you're seeing some bouncing. When Gilbert dropped into green numbers, his pancreas and liver panicked! He's not used to being back in normal range numbers. When this happens, glucagon and counterregulatory hormones are released and numbers spike back up. It can take roughly 72 hours for a bounce to clear.
I'm not sure what you're feeding Gilbert. Your SS notes that your recently discontinued dry food. I hope that someone directed you to Lisa Pierson, DVM's website on
feline nutrition and you're now giving Gilbert low carb (less than 10% carb), canned food.
Given that Gilbert was getting substantially more Novolin than his current dose of Lantus, it's possible that he's under dose at the moment. I would stick with increasing Gilbert's dose every 3 days unless his numbers aren't budging. If that's the case, we can fast-track the dosing IF you're available to monitor closely.
