My Gilbert has been on Lantus for 7 & 1/2 days. He was previously on Novolin for 3-4 wks w/ little success. He weighs around 13-14lbs...down from 16 when 1st got sick. I shot 1.25u 2 x day for 6 days...have increased to 1.5u today. Am I giving him enough? His #s for the majority are not coming down. Am I just asking for too much too early?
 
Hi Christy, welcome to LantusLand! Here is your last post. Patience is a virtue and absolutely a necessity here. It just takes a while for Lantus to have the full effect. Gilbert is doing fine for his first week. Could you please update the spreadsheet with today's data? I'm sorry, but we are pretty data focused around here. :roll: Ok, some would even say anal retentive. :lol: :lol: Please don't be shy about asking questions. There is an incredible amount of knowledge and experience here and people are quite willing to share.

Lantus is very different than Novolin. You're going to need to think very differently about insulin. While Novolin is potent, it is harsh. Lantus, on the other hand is less potent but far gentler and has considerably longer duration.

It looks like you are aware that Lantus dosing is based on the nadir and not on your pre-shot numbers. The initial dose is held for 5 - 7 consecutive days. Once past the initial week, doses are evaluated after 3 consecutive days unless a reduction is indicated based on a drop below 50. If you skip a shot, give a fur shot, etc., the count starts over since the dosing is no longer consecutive. If you've not already done so, you may want to look at the Tight Regulation Protocol sticky.

I have a couple of thoughts regarding Gilbert's spreadsheet (SS). First, you're seeing some bouncing. When Gilbert dropped into green numbers, his pancreas and liver panicked! He's not used to being back in normal range numbers. When this happens, glucagon and counterregulatory hormones are released and numbers spike back up. It can take roughly 72 hours for a bounce to clear.

I'm not sure what you're feeding Gilbert. Your SS notes that your recently discontinued dry food. I hope that someone directed you to Lisa Pierson, DVM's website on feline nutrition and you're now giving Gilbert low carb (less than 10% carb), canned food.

Given that Gilbert was getting substantially more Novolin than his current dose of Lantus, it's possible that he's under dose at the moment. I would stick with increasing Gilbert's dose every 3 days unless his numbers aren't budging. If that's the case, we can fast-track the dosing IF you're available to monitor closely.

Anne, thank you! I updated his SS a couple of hrs ago, but have noticed that it takes time seems like for it to actually hit the forum.

Sienne, thank you for the comforting words & info. I am still giving very little low carb dry & trying to increase his low carb wet. He is currently getting FF ckn & liver, chicken feast, & mixed grill. How many cans a day do you think he should be eating @ his weight?
 
Hi Christy....welcome to Lantus Land! I'm glad you found us.

Do you k ow Gilbert's ideal weight? You could start at 20 calories/pound; feed for a week and then weigh him. If he's gained, cut it back a tiny bit. If he's lost, increase it. You are just trying to fine tune enough calories to keep his weight stable.
 
Dr. Lisa's site gives a formula for estimating calories per day:
Required calories per day = [13.6 X optimal lean body weight in pounds] + 70​
So, if Gilbert's weight is good, he should be getting approx. 254 calories per day using a weight of 13.5 lbs. Dr. Lisa's food chart has the calories for what your feeding. The FF Classic Chicken Feast is 91 calories in a 3 oz can. So, Gilbert should be getting 2-3/4 cans of FF chicken per day.
 
I can see the updates to the spreadsheet now. :-D You are doing a great job with the testing but would it be possible to get a before bed test at night?

Thanks everyone! Really appreciate the tips on feeding. He has lost quite a bit of weight & don't want him losing any more. He was wasting away on his back & rear...backbone visible & backside sunk in, but since on insulin that seems to be getting better. I will try to get a +3 tonight...if you want a different time...let me know.
 
Unless Gilbert throws you a curve tonight, I would raise his dose tomorrow.

Just a housekeepng note: We open one condo (thread) per cat per day. If you start a new thread for Gilbert tomorrow, you'll want to have a subject line that includes his test data. The subject line reads: date --- kitty's name --- AMPS-#, +time-#.
 
The convention is to open one thread for each cat each day and to provide a link to the previous one. Then each thread doesn't get too long and advice givers can trace back to find previous history on the cat. It also keeps the board from getting too cluttered with too many threads and all the information about each cat is in one place. It works pretty well once you get used to it. :-D
