Hi all

After a "stable" few weeks with mostly yellow readings we felt we needed to try and drop Bailey's BG a little so upped the dose very slightly a week and a half ago aiming to get him under 10 at his nadir.

We had a few days of not much change lots of yellow numbers then followed by a dip at the nadir to 7 which was good but then a PMPS of 13 that day. The next day he was very high in the pinks and then a record high (on Lantus) with a PMPS of almost 20.

The next day he was back down again to 10 early morning AMPS (no change in dose during all of this) and what followed were a few days mostly in the blue and a distinctly perkier Bailey!

Today he started low again at 11 AMPS but then shot up with his +6 at 14 and his PMPS upto 18 and into the pink again so a day today of blue - yellow - pink.

We're a bit stumped as of course we have changed the dose and after a few days it seems to have caused a much wider swing whereas before he was very flat for most of the day.

If anyone has any thoughts we'd really appreciate it.

Thank you

Hi Barnes

Okay couple of things. 4u looks very close to an ideal dose for Sir Bailey but his numbers must have spooked you because you lowered it to 3.75 for a couple of shots that's what set the swing in motion. You've got a lot of wiggle room even at 7-8 for him to drop before he's dangerously low. Low end of normal is 3.8 So I think you still have room to take him up some. Ideally you'd like to see a sprinkling of green around nadir.

I think I would take him up again to either 4.25 or 4.5 Sometimes its just the quirky nature of a depot insulin that it isn't what you did last cycle so much as what happened a couple cycles back that influences today's numbers. And then sometimes the cause and effects are immediate depending on the cat.

Thanks Mel

As always!

You are quite right the low numbers do spook us and with not being able to test more often that is always in the back of our minds. He seemed really good yesterday staying in those blues all day we had our old Bailey back :)

We'll increase him and see how he settles after a few days. We just need to hold our nerve!

Thanks again we are eternally grateful for all your help.

lol I have to tell you after getting use to seeing numbers in the US scale then seeing the header with numbers in the teens it almost gave me a coronary until I realized it was in UK readings.

This is one of the few times I actually prefer the US scale over metrics. Because for you 2.2 is hypo and green starts at 5.5 For me 40 is hypo but green starts at 99. It's a lot easier on the nerves to shoot a 140 when you know you have 100 points of wiggle room than it is to shoot a 7.7 and only having 5.5 pts to wiggle in. lol

Sorry Mel I will have to start using the US scale so I don't give you any more scares! :)

We're going to up him as you suggest and see how that works whilst holding our nerve!

If we get another pre shot reading under 10 then should we delay the shot until he goes above 10? What's the general rule here? And what is a good/safe nadir reading to aim for?

Also do you think those pinks coming from out of all those blues could have been a rebound? Seems odd that he should shoot up like that considering he has been so flat even despite the dose change.

Thanks Mel you are a life saver!

Mel is a lifesaver, we would have never made it the first few months without her. She's been mine and Lucian's angel for almost a year. o:-)

Have to remember, anytime a cat goes lower that what his body is used to, he will most likely bounce high for a couple cycles, then settle back in. Used to scare me to death! Thankfully, Mel kept me cool and kept my head on straight until I learned a little something.

Good luck!

Sorry for not getting back yesterday in the midst of Holiday chaos. lol

But you're probably looking at wanting his nadirs in the mid 3s to low 4s (60-90ish)
