B12 Injections/Dosage?

What is the dosage of B12 injections for 12.5lb cat?
Smokie is a 17 years old diabetic with CRI. She is diet controlled. She is anemic now and
I need to tell my Vet how to write the prescription.

Also, where is the best place to purchase? I can get a 1ml vial locally for $13.00.
