Hi, new here to the forum. My cat mischief has acromegaly amd im giving him 7 units lantus twice a day. His sugars are always in high 400s , I check 6 hours after morning dose which is around 200's amd creeps back to 400s by his second dose. Today I checked him tonight and he was 213 so I havemt given him his night dose. Would you hold off?

Once set up you can post here. How long ago was he diagnosed? Was he DKA and do you test for ketones? Are you feeding a low carb wet or raw diet? Do you have a hypo kit with plenty of test strips, high carb food, and Kara or honey? That’s not a low pmps but since it is for you and not having more information it’s hard to advise you.

Without more data, it's hard to give a definitive answer but if he were my cat and I was able to test tonight (and had plenty of supplies like test strips, high and medium carb food, Karo/honey/syrup), I'd go ahead with the scheduled dose.

I'd be sure to get a +1 and +2 to see what happens (and if necessary, you can intervene early).

Hello Stephanie and super sweet Mischief. In addition to what the others say, you might want to vary the +6 test time you are doing every day. It's not uncommon for acrocats to nadir later, which it looks like he did today.

With a little bit more information, I hope we can help you get Mischief more regulated. We have quite a few cats here with acromegaly that are doing well.
