Hi everyone! :bighug:

A new member @Kaori living in Japan is seeking help. We've communicated on messages. She doesn't speak English, and she's using Google translator.

What I know up to now:

- Following a vet's advice Kaori started with 0.5 unit Lantus. Then increased to 0.75, 0.8, 0.9. Now her vet advice to increase to 1 unit.
Kaori has a fear since her kitty's BG drops sometimes. She wants to know if we'd agree on 1 unit.

- She's started to filling out the spreadsheet.
- She wants to follow SLGS.
- She is using a human meter.
- She doesn't know how to update signature.

@Kaori, we need to see your spreadsheet.

Also, is there anything I can do for her?
She also would like more Japanese friends on the board. By any chance are there more Japanese here? (myself I've never thought about it!)

Hello to Kaori and her kitty, Name? Please have her fill out a SS and we will do the best we can to help her. Tell her to post so we can see her and her kitty. So glad that you are here Kako in case we need any translations. :) My kids are Japanese but, I know hardly any Japanese. ( I can sing her a great nursery song or two, though, LOL )
 
As far as updating her signature, tell her to click on her name in the upper right hand corner of the page. It will open up to a window that will give her the option to click on her signature and then she can update it .
 
If there are some things in advance you would like translated, I have a Japanese friend who might be able to help. Would most likely be transliterated. Meaning こんにちは = konnichiwa. If REALLY necessary I can try my best to type it into Hirigana. But am I correct that Kako speaks and write Japanese?

Yong, yes, I speak and write Japanese.
Somehow Kaori's message is in English using Google translator. I write her back in Japanese with Japanese font, but her response is in English, and Google translator is often very funny...

@Kaori, you need to post to receive advice. Write here to start with. If Google translation works funny, I will re-translate.
Also, you are always to welcome to message me for questions.

And here is how to update your signature. (As Bobbie wrote)

chi chi pa pa chi chi pa. Little birds learning to sing by the sensai ( teacher)

So cute these songs and my adult sons still remember them!!

Do you know either of these?

I can't remember Ami Ami but I know "chi-chi-pappa chi-pappa Suzume (sparrow) no gakkou (school) no sensei (teacher) wa~~ :)
 
My son, Jamie just sent this to me after I asked if he remembered either song. I loved this song and sang them to my boys all the time.

I think it's safe to increase to 1u. Though I'm not experienced enough to give dose advice.

Utyun began insulin after discharge of ketoacidosis.
At the first hospital, he became hypoglycemic at 0.2 units of Levemir.
After that I started Lantus at a different hospital,
I was afraid to raise the dose, so I could not control it.
I thank you for listening to everyone's advice! Thank you very much!

Are you using U100 insulin syringes?

Are all the numbers on the SS with Lantus?
Or are some with Levemir?
When did he go hypo with Lev?
Are you checking his urine for ketones? With a history of ketoacidosis I would recommend that you check for ketones daily if possible.
Is Utyun, feeling ok, eating well?
How much urine is he producing?
Does he drink a lot?
What are you feeding is it all wet food? Low in Carbohydrate?
As kitties are prone to getting dehydrated wet food is much healthier for them and a lot of us here add water to the food as well, In Utyun's case adding water to his food would be additionally helpful as water helps to flush out any ketones, and this can help prevent another episode of diabetic ketoacidosis (DKA).
Can you please add DKA to the signature as a history of DKA alters the approach/advice for a kitty.

I'd like to point out a couple of things, that 590 this morning, makes me think he might be bouncing, bouncing is when a cat BG (blood glucose) drops fast or into lower numbers than that cat is used to, this triggers a reaction which ultimately results in the liver releasing glucose into the blood stream and results in high BG levels, the levels can remain high for upto 6 cycles, though sometimes less.

That 590 this morning looks suspiciously like Utyun, may have dropped lower last night, he dropped 150 points by +4, now that is fast enough that it may have triggered a bounce, but it is also possible that he continued to drop past +4 last night, if he did, we have no way of knowing how far or if he did.
When you find the numbers dropping significantly like that it's always a good idea to follow up with a test later in the cycle, (last night either an hour later, or set the alarm off to get a test say at +6 would have been very informative) , to see if he is still dropping or to see if his BG levels have leveled of.

No, at the beginning I did not have a home test meter.

It was January 2 after discharge.
After that, the first veterinarian could not insulin until abandoning treatment and finding the next hospital.

Drinking water became normal, but I think there are many urine.

We changed to canned low carbohydrates.
But in Japan there is no fancy feast, but instead there is monputi but the raw material seems to be different.
There are also Nature's Variety and Jiwi Peak but it is very expensive...

I'm so glad that you are here, there are so many wonderful and knowledgable people who can help you and your adorable Utyun manage his FD. Hopefully, they'll have him feeling better soon!

I will leave the detailed discussion to the more experienced people here, but I do have one question after looking at your spreadsheet. You have a lot of fractional doses-- 0.6, 0.7, 0.8, etc.-- that I simply can't imagine myself being able to measure consistently on a U-100 syringe. How are you doing this? Are you using calipers? Just to make sure we're all talking about the same insulin amounts, here is a picture of 0.75U (from the page on "Drawing and Handling Fine Doses"):

I was taught my friend Crack Major. Although it measures with a crack major, it seems that it is small and eyes are going crazy and I think there is some error.

I was taught my friend Crack Major. Although it measures with a crack major, it seems that it is small and eyes are going crazy and I think there is some error.

I'm afraid I don't understand what this is-- a google search for "Crack Major" redirected me to stories about the former Mayor of Toronto ;). I'm guessing it is actually some kind of caliper????

It is a plastic sheet with many scales (originally to measure cracks) and you can use it like calipers.

It is by millimeter. "1.00" is 1.00 millimeter.

Hello and welcome.
For people using BD Micro Fine 0.3 ml U-100 syringes, there is a paper printout that can be used to measure the scale. It was developed by the German Diabetes Katzen board. The description of how to use the paper scale is the paper called "Management of Diabetic Cats" which is attached to the Tight Regulation Protocol Sticky Note. There is a section in the paper on administering small doses of glargine.

I used calipers with my BD syringes, and I found that one unit was closer to 1.4 or 1.5 mm.
