9/6 Ms. Sonni and ProZinc

Hello,

I am posting for a friend of mine....her cat Sonni was diagnosed about 7-8 weeks ago with diabetes and started on ProZinc and told NOT to hometest. I got her to understand how important hometesting is and she jumped right in and is doing great. They have recently moved across country and have settled into their new home about 10 days ago.

I need some help with ProZinc.....my Zoe was on Lantus and I understand the dosing, etc...but I don't understand ProZinc at all. I have attached her spreadhseet....so if anyone could take a look and give me some suggestions about dose....I will really appreciate it!

Sonni also has IBD and Feline Asthma. She is taking and will be for the rest of her life, 10mg Pred daily. She eats low carb, grainfree FF only.

It looks like you guys are doing great. It's fantastic that she has a friend who she trusts who is knowledgable about FD and knows the importance of home testing.

Your ss looked better before the new vial of Prozinc. You've started getting inverse curves and higher numbers. You might be getting more oomph out of this vial, if it were me I would drop back a little bit and see if that doesn't correct the curve.

Prozinc can be a little unpredictable in the first 6 weeks. Nadir can move around some and the numbers can pinball some for no apparent reason, but seems to settle down after about 45 days. I've also discovered, at least with Harley, that small dose changes like .1 or .2u can give big results so I have stayed away from .5u changes (unless to drop the dose if too high).

Are there any specific questions? Others will be by soon.

So I should have her decrease the dose? and how do you decrease .1 or .2? She has U40 syringes.

Are you familiar with the conversion to U100 syringes?

U100 syringes are much easier to micro dose. I use 3/10mL, 30 unit, 30 guage, 8mm (although I prefer a longer needle) with 1/2 unit markings. BUT YOU NEED to use the conversion chart with it. I printed it out and hung it on the fridge and check it twice before every shot. If you need, I can go find it for you. There are pictures here too if you do a search, of what small doses look like in a syringe.

I guess I would shave a drop or so off of the .5u. Try to measure it just under the .5 unit. The pictures helped me alot.

With Harley a while back 1.2u was almost perfect but not quite so I upped him to 1.4 and quickly realized that was too much.

You can drop it quicker that that but I've learned when I over react and drop it too much it sets us back a few steps. Then we have to waste a couple of days catching up.

I think the conversion chart is in the PZI sticky up top, and some info on which syringes to get.

It looks to me like you have a working pancreas (see 8/6) so I'm guessing that's part of what is making the #s confusing. No U-curves really, but then some of that could be her pancreas doing some of the work. I'd be tempted to try 0.25 for a few days and see what happens. If you saw a nice drop down to a green # around +6ish, that might confirm the dose is too high now. Since the #s were running higher on the 0.75 it's hard to tell what is cause & what's effect. I'm thinking with a sputtering P maybe the insulin is almost interfering with things? Some PZI cats respond amazingly to a micro dose - others it'll be shooting water. If it were me, I'd probably give it a shot (0.2 or so) and see.
 
I have been working with this person too. One thing to note is that during the span of all the inverse curves, they were living in a friend's house and the cats were confined to one room and weren't too happy about it.

My advice last time we spoke was that there is either too much insulin or too little insulin. The only way to test that out would be to do 0.25u for a cycle or two and see where that takes Sonni. She is not prone to ketones, so it should be safe to do for a few cycles.
 
Thanks, Kelly....I forgot to include about living in one room! I did mention to her this morning about dropping back to 0.25 for a few days and see how Sonni does.
