I felt really encouraged by Harvey's second day at 6.5 units where he spent a good part of the day in the upper 200s, which is good by our standards.

Yesterday though he got into the 200s only once during that day (at +4). Last night we bumped him up to 7 units. He got down around 250 a couple times in the middle of the night, and he's been in the upper 200s for most of today. I've been here before though, and it doesn't seem to last or get any better.

We go back to the vet tomorrow evening for placement of his next Libre CGM device. I plan on asking her to test him for acromegaly and insulin auto antibodies. Is there anything else I should bring up to her?

I'm especially interested in acromegaly. I've done a little Googling, and I see that one symptom can be enlargement of the jaw. I've noticed that Harvey grinds his teeth A LOT when he's eating. I brought it up to the vet a few weeks ago and she said that sometimes diabetic cats do that. But I'm wondering if maybe they do it because the changes in their jaws affect their teeth alignment.

Of course, it could just be something going on in his mouth. He hasn't had a dental cleaning in several years because he has a heart condition, and we've been trying to balance the good of the dental against the risk of the anesthesia. In fact, he had just had an echocardiogram a couple days before he got diagnosed with diabetes.
 
Look at that looooong stretch of yellows! :) That's a good sign of progress.

Can't remember if I've asked before, but does the vet think he might need more than a dental cleaning? Neko had HCM and a heart murmur, but her echo still cleared her for anaesthesia, though I chose to go with a dental specialist because of the extra monitoring, and they were used to special needs animals.

By the way on the signs and symptoms of acromegaly, many cats show no acro symptoms at all at the time of diagnosis. Neko's only sign (other than higher dose and hunger), was one tearing eye - which I found out later was caused by soft tissue growth in her tear duct. Pretty subtle sign.

When you say more than a dental cleaning, do you mean extractions? His teeth are definitely gross and in need of cleaning. It's just been on the back burner, first because of the pandemic and now this whole diabetes thing.
